【目的】探讨分娩球配合自主体位助产对初产妇分娩时主观感受和分娩结局的影响。【方法】选取初产妇73例，根据产妇意愿分为两组：观察组产妇在分娩球配合自主体位下助产，对照组产妇采用传统半卧位或卧位待产，两组均由家属及导乐护士陪伴。分娩后采用问卷量化表对两组产妇分娩疼痛、控制感、焦虑程度等主观感受进行评估，并比较两组产程时长、产后2 h出血量及新生儿Apgar评分。【结果】观察组分娩疼痛、焦虑程度和第一产程时长均显著低于对照组，而分娩控制感显著高于对照组，差异均具有统计学意义（ P ＜0．05）；而两组第二、三产程时长，产后2 h出血量和新生儿Apgar评分比较无显著性差异（ P ＞0．05）。【结论】分娩球配合自主体位助产能够提高初产妇主观舒适感受，并缩短第一产程，值得临床推广应用。
【목적】탐토분면구배합자주체위조산대초산부분면시주관감수화분면결국적영향。【방법】선취초산부73례，근거산부의원분위량조：관찰조산부재분면구배합자주체위하조산，대조조산부채용전통반와위혹와위대산，량조균유가속급도악호사배반。분면후채용문권양화표대량조산부분면동통、공제감、초필정도등주관감수진행평고，병비교량조산정시장、산후2 h출혈량급신생인Apgar평분。【결과】관찰조분면동통、초필정도화제일산정시장균현저저우대조조，이분면공제감현저고우대조조，차이균구유통계학의의（ P ＜0．05）；이량조제이、삼산정시장，산후2 h출혈량화신생인Apgar평분비교무현저성차이（ P ＞0．05）。【결론】분면구배합자주체위조산능구제고초산부주관서괄감수，병축단제일산정，치득림상추엄응용。
[Objective] To explore the subjective perceptions and delivery outcomes of free posture assisted by birth ball on primipara .[Methods] A total of 73 primiparas were divided into two groups by their inten‐tions .The experimental group adopted a free posture assisted by birth ball and were accompanied by their hus‐bands and a nurse .And the control group assumed a traditional semireclining or decubitus position .After childbirth ,the subjective perceptions of pain ,control and anxiety were evaluated by questionnaires .And birth process time ,volume of bleeding within 2 h and Apgar score were also evaluated .[Results] The degrees of pain ,anxiety and duration of first‐stage labor were lower in experimental group than those in control group ( P<0 .05) .And the degree of control feeling was higher in experimental group than that in control group ( P<0 .05) .No significant inter‐group differences existed in duration of second/third‐stage labor ,volume of bleed‐ing within 2 h or Apgar score ( P>0 .05) .[Conclusion]Free posture assisted by birth ball can bring comforts and shorten the time of first‐stage labor for primiparas .
