目的：探讨标准四联疗法对老年幽门螺杆菌（HP）阳性的消化性溃疡患者的疗效及安全性。方法将老年HP阳性消化性溃疡患者128例选为老年组，同期确诊的中青年HP阳性消化性溃疡患者268例选为中青年组。2组患者均给予标准四联疗法，即奥美拉唑肠溶片20 mg，阿莫西林胶囊1 g，克拉霉素分散片0.5 g，枸橼酸铋钾片0.22 g，均口服2次/日，疗程14 d。结果 HP 根治率老年组（83.59％）与中青年组（85.07％）相比无显著性差异（χ2＝0.1458，P＝0.7046）。老年组6周内胃溃疡愈合率为74.07％，显著低于中青年组的89.66％，( P＜0.05）。老年组4周内十二指肠球部溃疡愈合率为63.64％，显著低于中青年组的79.41％( P＜0.05）。老年组不良反应发生率为39.84％，显著高于中青年组的22.39％(χ2＝13.0842，P＝0.0003），便秘及腹泻的发生率为6.25％和8.59％，均显著高于中青年组的1.49％和2.99％( P＜0.05）。所有并发症均经对症处理后消失，不影响继续用药。结论标准四联疗法对HP阳性的老年消化性溃疡患者疗效肯定，不良反应轻微，耐受性较好。
目的：探討標準四聯療法對老年幽門螺桿菌（HP）暘性的消化性潰瘍患者的療效及安全性。方法將老年HP暘性消化性潰瘍患者128例選為老年組，同期確診的中青年HP暘性消化性潰瘍患者268例選為中青年組。2組患者均給予標準四聯療法，即奧美拉唑腸溶片20 mg，阿莫西林膠囊1 g，剋拉黴素分散片0.5 g，枸櫞痠鉍鉀片0.22 g，均口服2次/日，療程14 d。結果 HP 根治率老年組（83.59％）與中青年組（85.07％）相比無顯著性差異（χ2＝0.1458，P＝0.7046）。老年組6週內胃潰瘍愈閤率為74.07％，顯著低于中青年組的89.66％，( P＜0.05）。老年組4週內十二指腸毬部潰瘍愈閤率為63.64％，顯著低于中青年組的79.41％( P＜0.05）。老年組不良反應髮生率為39.84％，顯著高于中青年組的22.39％(χ2＝13.0842，P＝0.0003），便祕及腹瀉的髮生率為6.25％和8.59％，均顯著高于中青年組的1.49％和2.99％( P＜0.05）。所有併髮癥均經對癥處理後消失，不影響繼續用藥。結論標準四聯療法對HP暘性的老年消化性潰瘍患者療效肯定，不良反應輕微，耐受性較好。
목적：탐토표준사련요법대노년유문라간균（HP）양성적소화성궤양환자적료효급안전성。방법장노년HP양성소화성궤양환자128례선위노년조，동기학진적중청년HP양성소화성궤양환자268례선위중청년조。2조환자균급여표준사련요법，즉오미랍서장용편20 mg，아막서림효낭1 g，극랍매소분산편0.5 g，구연산필갑편0.22 g，균구복2차/일，료정14 d。결과 HP 근치솔노년조（83.59％）여중청년조（85.07％）상비무현저성차이（χ2＝0.1458，P＝0.7046）。노년조6주내위궤양유합솔위74.07％，현저저우중청년조적89.66％，( P＜0.05）。노년조4주내십이지장구부궤양유합솔위63.64％，현저저우중청년조적79.41％( P＜0.05）。노년조불량반응발생솔위39.84％，현저고우중청년조적22.39％(χ2＝13.0842，P＝0.0003），편비급복사적발생솔위6.25％화8.59％，균현저고우중청년조적1.49％화2.99％( P＜0.05）。소유병발증균경대증처리후소실，불영향계속용약。결론표준사련요법대HP양성적노년소화성궤양환자료효긍정，불량반응경미，내수성교호。
Objective To exPlore the theraPeutic effects and safety of standard quadruPle theraPy for elder PePtic ulcer Patients with Positive of Helicobacter Pylori(HP). Methods 128 elder PePtic ulcer Patients with HP infection were considered as elder grouP and 268 young and middle-aged PePtic ulcer Patients with HP Positive were regarded as young and middle-aged grouP. The Patients in the 2 grouPs were treated by standard quadruPle theraPy(omePrazole 20 mg,amoxicillin 1 g,clarithromycin 0. 5 g,oral 0. 22 g,2 times/d), and 14 d was 1 course of treatment. The HP eradication rate,ulcer healing rate,and drug adverse reactions were comPared between the two grouPs. Results The HP eradication rate in elder grouP(83. 59%)was not different comPared with young and middle-aged grouP (85. 07%, χ2=0. 145 8,P=0. 704 6). The gastric ulcer healing rate after treated for 6 weeks in elder grouP (74. 07%)was lower than young and middle-aged grouP (89. 66%, P < 0. 05). The duodenal ulcer healing rate after treated for 4 weeks in elder grouP (63. 64%)was lower than young and middle-aged grouP(79. 41%, P < 0. 05). The incidence rate of drug adverse reactions in elder grouP (39. 8%)was increased than young and middle-aged grouP (22. 39%, χ2=13. 084 2,P=0. 000 3),the incidence rate of constiPation and diarrhea in elder grouP (6. 25% ,8. 59% )was significantly higher than young and middle-aged grouP (1. 49% , 2. 99%, P < 0. 05). All the drug adverse reactions were disaPPeared after treating and did not imPacted continue treatment. Conclusion The quadruPle theraPy for elder ePticulcer Patients with Positive of HP is effective,has less adverse effects and better survivability.
