目的 探讨白芍总苷( TGP)治疗对原发性干燥综合征( pSS)患者外周血浆细胞样树突状细胞( pDC)的影响. 方法 收集新发pSS患者17例作为疾病组,每日予以TGP 1. 8 g治疗3个月,对照组17例. 采用流式细胞术检测疾病组治疗前、后及对照组pDC及其表面CD32、CD86、CXCR4和Blys表达;ELISA法检测血浆干扰素-α( IFN-α)、白细胞介素-6 (IL-6)、肿瘤坏死因子-α(TNF-α)水平;免疫组化法检测25例pSS患者唇腺pDC浸润. 结果 ① pSS患者外周血pDC比例较对照组下降,pDC表达CXCR4比例较对照组增加,血浆IL-6、TNF-α、IFN-α水平较对照组上升,25 例pSS患者唇腺中19例有pDC浸润;②TGP治疗后,pSS患者外周血pDC比例较治疗前上升;pDC表达CXCR4、Blys比例较治疗前下降;TGP治疗后血沉下降;血浆 IL-6、TNF-α 水平治疗后下降. 结论 新发pSS患者较正常者外周血pDC比例降低,表达CXCR4增加,唇腺组织中有pDC浸润;TGP可能抑制pSS患者pDC活化.

목적 탐토백작총감( TGP)치료대원발성간조종합정( pSS)환자외주혈장세포양수돌상세포( pDC)적영향. 방법 수집신발pSS환자17례작위질병조,매일여이TGP 1. 8 g치료3개월,대조조17례. 채용류식세포술검측질병조치료전、후급대조조pDC급기표면CD32、CD86、CXCR4화Blys표체;ELISA법검측혈장간우소-α( IFN-α)、백세포개소-6 (IL-6)、종류배사인자-α(TNF-α)수평;면역조화법검측25례pSS환자진선pDC침윤. 결과 ① pSS환자외주혈pDC비례교대조조하강,pDC표체CXCR4비례교대조조증가,혈장IL-6、TNF-α、IFN-α수평교대조조상승,25 례pSS환자진선중19례유pDC침윤;②TGP치료후,pSS환자외주혈pDC비례교치료전상승;pDC표체CXCR4、Blys비례교치료전하강;TGP치료후혈침하강;혈장 IL-6、TNF-α 수평치료후하강. 결론 신발pSS환자교정상자외주혈pDC비례강저,표체CXCR4증가,진선조직중유pDC침윤;TGP가능억제pSS환자pDC활화.
Objective To investigate the effect of TGP on pDCs in new pSS patients and its mechanism of anti-in-flammatory effects. Methods 17 new pSS without any treatment were recruited as disease group, accepted total glucosides of paeony for 3 months, and 17 healthy subjects as control group. The percentages of peripheral blood pDCs,circulating plasma IFN-α,IL-6,TNF-α levels were analysed by Flow cytometry and ELISA. Immunohisto-chemistry method was used to observe the pDCs infiltration in labial gland. Results New pSS patients had signifi-cantly lesser pDCs in PB,and CXCR4 expressed by pDCs were higher than healthy controls. TGP could inhibit pDC activation and then reduce CXCR4 expression. Plasma IFN-α,IL-6,TNF-α levels of new pSS were increased, and TGP reduced IL-6,TNF-α levels. Moreover, pDCs were present in labial gland of pSS under the effect of chemo-kines receptor CXCR4 . Conclusion The findings reveal that activated pDCs transfer to the labial gland under the effect of chemokines receptor CXCR4 . Total glucosides of paeony inhibit the activation of pDCs in primary Sj?gren′s syndrome.
