目的 了解睑脂的种类和成分,并分析比较健康者和睑板腺功能障碍(MGD)患者的睑脂成分.方法 病例对照研究.正常组20例,MGD组19例,均采用改良的睑板腺压榨方法收集睑脂,采用GC-MS法分析睑脂成分与结构.条件:色谱柱为DB-5 MS(30 m×0.25 mmX0.25 μm)毛细管柱,载气为He,流速1.0 ml/min;进样量:1 ml.质谱条件:质谱电离方式为电子电离(EI),电子轰击源电子能量70 eV,离子源温度200℃;电子倍增器电压1.10 kV;溶剂延迟时间:1.00 min;扫描方式:san,扫描范围m/z:20~500 aum.通过E1质谱碎片和数据库中所存储的标准物质的质谱信息的比对的方式进行鉴定.采用色谱峰面积归一化法,计算其组分的相对含量.各睑脂成分相对含量数据用中位数和四分位数间距表示,采用非参数检验进行统计学比较.结果 通过GC-MS系统,共分离出102种睑脂成分,明确鉴定23种化合物,包括3种烷烃、4种烯、2种醇、1种芳烃、7种酯、4种脂肪酸、1种酰胺、1种酚,尚有79个色谱峰因为质谱图库中无相应物质信息,未进行对比鉴定.睑脂成分的个体差异性较大.其中软脂酸、硬脂酸、角鲨烯水平在MGD患者组明显低于正常组(U软脂酸=-2.086,U硬脂酸-2.027,U角鲨烯=-2.866;P<0.05).结论 睑脂成分种类繁多,结构复杂,个体差异大,其中软脂酸、硬脂酸、角鲨烯在MGD患者中明显降低,提示MGD与睑脂成分改变有关.

Objective To analyze and identify the meibum from healthy individuals and patients with meibomian gland dysfunction (MGD) by gas chromatography-mass spectrometry (GC-MS).Methods Case-control study.The meibum from twenty healthy persons and nineteen MGD patients was separated and analyzed by GC-MS.GC conditions were:column:DB-5 MS (30 m × 0.25 mm × 0.25 μm) capillary column,carrier gas:He,flow rate:1.0 ml/min,injection amount:1 ml.MS conditions were:MS ionization mode:EI,electron impact source electron energy:70 eV,ion source temperature:200 ℃,electron multiplier voltage of 1.10 kV,solvent delay time:1.00 min,scanning mode:San,scanning range m/z 20-500 aum.The components of meibum were identified by E1 mass fragmentation and the standard substance in the database.The relative content of the components was calculated by the method of chromatographic peak area normalization.Results In the 39 samples that were analyzed by GC-MS,102 chromatography peaks were separated,and 23 compounds were identified,including 3 alkanes,4 alkenes,2 alcohols,1 aromatic hydrocarbon,7 esters,4 fatty acids,1 amide,and 1 phenol.However,there was no corresponding physical mass library information in the remaining 79 GC peaks,and no identification could be performed.The relative concentration level of palmitic acid,stearic acid,and squalene in the MGD group was significantly lower than that in the healthy controls (U=-2.086,-2.027,-2.866,P<0.05).Conclusions Meibum has a wide variety of components,the structures are complex,and the components in different individuals vary greatly.Palmitic acid,stearic acid,and squalene levels were significantly decreased in MGD patients compared to the healthy group,suggesting that MGD is related to changes of the meibum components.
