目的:探讨急性脑梗死患者发病24 h 内外周血白细胞及亚型细胞的变化特点。方法:急性脑梗死患者96例,统计其外周血白细胞及其亚型细胞变化,并记录美国国立卫生研究院卒中量表(NIHSS)评分,DWI 测定脑梗死体积,利用受试者工作特征(ROC)曲线及 Logistic 回归分析模型进行统计。结果:脑梗死急性期外周血中性粒细胞升高相比白细胞总数升高及淋巴细胞减低更为明显(P<0.05),且发病时间≥3.5 h、NIHSS 评分≥3.5分与外周血中性粒细胞升高明显相关(P<0.05)。结论:脑梗死初期外周血中性粒细胞升高可能有助于推测脑梗死可能的发生时间及严重程度。

목적:탐토급성뇌경사환자발병24 h 내외주혈백세포급아형세포적변화특점。방법:급성뇌경사환자96례,통계기외주혈백세포급기아형세포변화,병기록미국국립위생연구원졸중량표(NIHSS)평분,DWI 측정뇌경사체적,이용수시자공작특정(ROC)곡선급 Logistic 회귀분석모형진행통계。결과:뇌경사급성기외주혈중성립세포승고상비백세포총수승고급림파세포감저경위명현(P<0.05),차발병시간≥3.5 h、NIHSS 평분≥3.5분여외주혈중성립세포승고명현상관(P<0.05)。결론:뇌경사초기외주혈중성립세포승고가능유조우추측뇌경사가능적발생시간급엄중정도。
Objective: To investigate the dynamic changes of peripheral total leukocytes, and subtype cells in patients with stroke onset within 24 h. Methods: Ninty-six patients diagnosed as newly onset of cerebral infarction were enrolled. The clinical data including the counts of the total leukocytes and the subtypes in peripheral blood, NIHSS score, and the infarction volume in DWI were analyzed. The potential factors involved in the changes of pe-ripheral blood leukocytes were analyzed by Logistic regression analysis and receiver operating characteristic (ROC) curves. Results: The abnormal rising of the counts of neutrophils in peripheral blood was more remarkably than that of the total leokocytes and the lymphocytes (P<0.05). The time of stroke onset ≥3.5 h and the NIHSS score≥3.5 may be the main risk factors of the abnormal increase of neutrophils (P<0.05) in peripheral blood within 24 h after stroke onset. Conclusion: The increase of peripheral blood neutrophils in the early stage of cerebral infarction may be helpful to evaluate the duration and severity of cerebral infarction.
