目的:评价同伴教育对社区2型糖尿病患者自我管理的影响.方法采用 Meta 分析的办法,检索多个中英文数据库,搜集相关临床随机对照试验和半随机对照实验,进行文献质量评价后,采用 RevMan 5.0软件进行数据分析.结果共纳入9篇文献,同伴教育干预组的自我管理能力提高效果明显高于对照组[WMD=1.20,95%CI (0.71,1.68),P<0.00001];在 HbA1c方面,对不同干预周期做亚组分析,结果显示干预时间>6个月的同伴教育能显著降低 HbA1c[WMD=-1.43,95%CI (-1.74,-1.12),P<0.00001],而干预时间<6个月的同伴教育与对照组相比,没有显著性差异[WMD=-0.24,95%CI (-0.48,0.01),P=0.06].结论同伴教育在提高2型糖尿病患者自我管理能力上显示出优越性,同时长期的同伴教育(>6个月的)对 HbA1c的降低效应显著高于传统健康教育方法,但短期的同伴教育(<6个月)对 HbA1c 的降低效果还有待于临床开展多中心、大样本随机对照试验来证实.

Objective To evaluate the efficacy of peer education for type 2 diabetes patients in community on self-management.Methods By using meta analysis,several Chinese and English databases were searched,and to collect relevant clinical randomized controlled trials (RCTs)and quasi-RCTs,and the data were processed with RevMan 5.0.Results A total of nine trials were included.Compared with the control group,the intervention of peer education can significantly improve the self-management ability of type 2 di-abetes patients[WMD=1.20 ,95%CI (0.71 ,1.68),P<0.00001].The subgroup analysis showed long-term intervention of peer education(beyond 6 months)was more effective in decreasing HbA1c[WMD=-1.43 ,95%CI (-1.74,-1.12),P<0.00001],but the advantage did not present for relative short-term interven-tion (within 6 months)[WMD=-0.24,95%CI (-0.48,0.01),P=0.06].Conclusion Peer education is as-sociated with better self-management results among type 2 diabetes patients and long-term intervention of peer education (beyond 6 months)was more effective in decreasing HbA1c.However,multicentric large-sample randomized controlled trials of higher quality are needed to confirm short-term intervention of peer education (less than 6 months).
