目的：探讨术前使用呼吸功能锻炼器进行训练对促进中老年肝脏手术患者康复的效果.方法便利选取2013年11月至2014年11月东方肝胆外科医院收治的50~70岁的肝脏手术患者74例，采用随机数字表法分为试验组和对照组各37例.对照组仅给予常规术前准备，试验组在常规术前准备的基础上使用呼吸功能锻炼器进行训练，比较两组患者术前肺功能、术后呼吸系统并发症及肝功能的差异.结果两组患者在年龄、性别构成、体质指数、入院时肺功能评估、手术方式、术中出血量等方面的差异均无统计学意义(均P＞0．05)；试验组患者术前屏气试验时间长于对照组，术后肺不张发生率低于对照组，术后7 d天门冬氨酸氨基转移酶低于对照组，差异均有统计学意义(均P＜0．05).结论中老年肝脏手术患者术前利用呼吸功能锻炼器进行训练，有助于改善术前肺功能、降低术后肺不张的发生率、促进术后肝功能恢复，从而提高患者的术后康复效果.

Objective To discuss the effect of respiratory function exercise equipment in improving the re-habilitation effect of elderly patients receiving hepatectomies.Methods By convenience sampling,74 pa-tients were selected and equally divided into experimental group and control group.The control group re-ceived conventional preoperative preparation,and the experimental group was applied with respiratory function exercise equipment on the basis of routine preparation.The preoperative pulmonary function,post-operative respiratory system implication and the difference of pulmonary function were compared.Results There was no statistical significance showed in two groups of patients regarding to age,gender,constitu-tional index,pulmonary function assessment,operation type and blood loss during operation (all P>0.05). The breath-holding time in experimental group was superior than which in control group,and the inci-dence of postoperative atelectasis was less than that of control group,the aspartate amino transferase of 7 days after operation in experimental group was lower than which in control group (all P<0.05).Conclu-sion The application of respiratory function exercise equipment in the preoperative exercises can effective-ly improve the pulmonary function,reduc the incidence of atelectasis,promot the postoperative pulmonary function recovery,thus to improve the rehabilitation effect.
