1例44岁女性哮喘患者，长期未规范治疗。1年前因哮喘急性发作使用射流雾化吸入布地奈德混悬液治疗，出现声嘶、咽喉疼痛等症状，考虑为吸入糖皮质激素引起的局部不良反应。2个月前，痰真菌培养示黄曲霉阳性，纤维支气管镜检查见会厌声带有明显的白斑附着。临床医师和临床药师讨论后考虑有吸入糖皮质激素所致上气道真菌感染可能，给予口服伏立康唑（0.2 g、q12h）治疗，患者声嘶、咽喉疼痛等症状明显好转。
1례44세녀성효천환자，장기미규범치료。1년전인효천급성발작사용사류무화흡입포지내덕혼현액치료，출현성시、인후동통등증상，고필위흡입당피질격소인기적국부불량반응。2개월전，담진균배양시황곡매양성，섬유지기관경검사견회염성대유명현적백반부착。림상의사화림상약사토론후고필유흡입당피질격소소치상기도진균감염가능，급여구복복립강서（0.2 g、q12h）치료，환자성시、인후동통등증상명현호전。
A 44-year-old woman with asthma was treated with jet atomization inhalation of budesonide because of acute asthma attack a year ago. She was suffered hoarseness and throat pain after inhaled budesonide. It was the adverse drug reaction of inhaled corticosteroids (ICS). Sputum culture test showed Aspergillus flavus (+) and fiber bronchoscope showed epiglottis-vocal-cord was adhered with leukasmus 2 months ago. The clinician and clinical pharmacist considered that was upper-airway aspergillosis caused by ICS after discussed. So voriconazole (0.2 g, q12h, po) was given. The symptoms such as hoarseness and throat pain were significantly improved after drug treatment.
