目的:探讨重症肺结核机械通气患者中应用高能量营养乳剂肠内营养支持(EN)的效果。方法将64例行机械通气的重症肺结核患者随机分为观察组与对照组,各32例。对照组予以普通营养液能全力进行EN,观察组应用高能量密度营养乳剂瑞高进行EN,比较两组治疗前后的呼吸功能指标与血营养指标。结果 EN治疗后观察组的血液营养指标(IgA、IgG、TLC、PA、ALB及TLC)均显著高于对照组(<0.05),呼吸功能指标(PCO2、VCO2及VE)均显著低于对照组(<0.05);观察组的机械通气时间与住ICU时间均较对照组显著缩短(<0.05)。结论在重症肺结核机械通气患者中应用高能量营养乳剂进行EN能够改善患者的营养状态与呼吸功能,缩短机械通气时间与住ICU时间,改善临床预后,值得推广应用。

Objective To investigate the ef icacy of enteral nutrition (EN) of high-energy density nutrition emulsions in treatment of severe pulmonary tuberculosis patients with mechanical ventilation. Methods 64 severe pulmonary tuberculosis patients with mechanical ventilation were randomly divided into observation group and control group, 32 cases in each. The control group received general nutrition EN, the observation group received high-energy density nutrition emulsions EN,the Respiratory function parameters and blood nutrition indicators of the two groups were compared. Results After treatment observation group EN blood nutrition indicators (IgA, IgG, TLC, PA, ALB and TLC) were significantly higher ( <0.05), respiratory function parameters (PCO2, VCO2 and VE) were significantly lower than the control group ( <0.05);duration of mechanical ventilation and ICU stay in observation group was significantly shorter than the control group ( <0.05). Conclusion Hhigh-energy density nutrition emulsions EN in treatment of severe pulmonary tuberculosis patients with mechanical ventilation can improve the nutritional status and respiratory function in patients with shortened duration of mechanical ventilation and ICU stay, improved clinical outcomes, should be widely applied.
