目的：探讨血清脂联素在糖尿病合并胆囊结石形成过程中的作用，以及血清脂联素与糖化血红蛋白、血脂水平的关系。方法从入院的2型糖尿病患者中选取单纯糖尿病患者（ D组，29例）与糖尿病合并胆囊结石患者（ D＋G组，27例），并随机选取34例正常健康体检者（对照组）作为对照。所有受试对象均在禁食10～12 h后的试验日清晨抽取空腹静脉血3 ml，置于试管中，立即室温3000 r／min离心10 min，分离血清后进行相关检测。运用单因素方差分析法对各组间患者的年龄（ Age）、病程（ Duration）、体重指数（ BMI）、糖化血红蛋白（ HbA1c）的含量、血脂（ Blood-fat）[三酰甘油（ TG）、总胆固醇（ CH）、高密度脂蛋白（ HDL）、低密度脂蛋白（ LDL）]含量水平进行比较；运用多元相关回归分析血清脂联素与各因素之间的关系。结果（对照组）的血清脂联素水平高于2型糖尿病患者（D组）（t＝4 g．28， P ＜0．05）；对照组的血清脂联素水平高于2型糖尿病合并胆囊结石患者（D＋G组）（t＝4．83， P ＜0．05）；D组的血清脂联素水平高于D＋G组（t＝0．54， P ＜0．05）。血清中脂联素的含量与高密度脂蛋白（HDL）的含量呈正相关（ r ＝0．90）；血清中脂联素的含量与患者的BMI和HbA1c的百分比呈负相关（ r ＝－0．77和－0．72）。结论糖尿病患者易并发胆囊结石与其低血清脂联素水平有关；糖尿病合并胆囊结石患者血清脂联素的含量与BMI、血清中HbA1c的百分比以及HDL的含量有关。
목적：탐토혈청지련소재당뇨병합병담낭결석형성과정중적작용，이급혈청지련소여당화혈홍단백、혈지수평적관계。방법종입원적2형당뇨병환자중선취단순당뇨병환자（ D조，29례）여당뇨병합병담낭결석환자（ D＋G조，27례），병수궤선취34례정상건강체검자（대조조）작위대조。소유수시대상균재금식10～12 h후적시험일청신추취공복정맥혈3 ml，치우시관중，립즉실온3000 r／min리심10 min，분리혈청후진행상관검측。운용단인소방차분석법대각조간환자적년령（ Age）、병정（ Duration）、체중지수（ BMI）、당화혈홍단백（ HbA1c）적함량、혈지（ Blood-fat）[삼선감유（ TG）、총담고순（ CH）、고밀도지단백（ HDL）、저밀도지단백（ LDL）]함량수평진행비교；운용다원상관회귀분석혈청지련소여각인소지간적관계。결과（대조조）적혈청지련소수평고우2형당뇨병환자（D조）（t＝4 g．28， P ＜0．05）；대조조적혈청지련소수평고우2형당뇨병합병담낭결석환자（D＋G조）（t＝4．83， P ＜0．05）；D조적혈청지련소수평고우D＋G조（t＝0．54， P ＜0．05）。혈청중지련소적함량여고밀도지단백（HDL）적함량정정상관（ r ＝0．90）；혈청중지련소적함량여환자적BMI화HbA1c적백분비정부상관（ r ＝－0．77화－0．72）。결론당뇨병환자역병발담낭결석여기저혈청지련소수평유관；당뇨병합병담낭결석환자혈청지련소적함량여BMI、혈청중HbA1c적백분비이급HDL적함량유관。
Objective To investigate the role of serum adiponectin in the pathogenesis of gallstones,and to explore the correlation between serum adiponectin and glycosylated hemoglobin as well as blood fat levels.Methods The patients with simple diabetes mellitus were selected from the patients with type 2 diabetes mellitus as group D ( n =29),and the patients with diabetes mellitus complicated by gallstone were selected as group D+G ( n =27),and the other 34 healthy subjects were served as control group.The fasting vein blood specimens (3ml) were taken in all the subjects at 10~12 hours after fasting, then the related parameters were detected.The one-factor analysis of variance was used to compare these parameters including patient’s age,disease duration,BMI,HbA1c content,the levels of blood-fat,TG,,CH,HDL,LDL among the three groups, moreover, multiple correlation regression was used to analyze the correlation between serum adiponectin and these parameters. Results The serum levels of adiponectin in control group were significantly higher than those in group D ( P <0.05),which were also significantly higher than those in group D+G ( P <0.05).Moreover, the serum levels of adiponectin in group D were significantly higher than those in group D+G ( P <0.05).The adiponectin content in serum was positively correlated to that of HDL ( r =0.90),however,which was negatively related with BMI and the percentage of HbA1c ( r =-0.77, r =-0.72,respectively) .Conclusion The trend that the patients with diabetes mellitus are liable to be complicated by gallstone is related with the low levels of serum adiponectin,furthermore, the serum levels of adiponectin in patients with type 2 diabetes mellitus complicated by gallstone are correlated to BMI, percentage of HbA1c,HDL content.
