目的 探讨急性脑梗死(ACI)患者信号传导及转录激活子(STAT)-4、STAT-6水平与神经功能缺损以及脑梗死体积的相关性.方法 选择ACI患者122例为观察对象,采用酶联免疫法(ELISA)和免疫透射比浊法测定STAT-4、STAT-6和超敏C反应蛋白(hs-CRP)水平,与同期健康体检者74例对照比较.按照多田公司计算ACI患者脑梗死体积,分为大、中、小梗死组,根据神经功能缺损程度评分标准分为轻、中、重度损伤组,比较不同梗死体积与损伤程度时STAT-4、STAT-6和Hs-CRP水平的变化,并进行STAT-4、STAT-6与Hs-CRP之间的直线相关分析.结果 ACI患者血清STAT-4和hs-CRP水平明显高于对照组[(225.82±113.39)、(16.86±4.17) ng/L比(107.72±75.18)、(6.27±1.15) ng/L],STAT-6明显低于对照组[(305.89±117.26)ng/L比(421.65±107.76) ng/L],差异有统计学意义(P<0.05).脑梗死体积越大,神经功能缺损程度越重,ACI患者血清中STAT-4和Hs-CRP水平越高,而STAT-6越低.血清STAT-4与hs-CRP水平正相关,而STAT-6为负相关(P<0.05).结论 ACI患者STAT-4、STAT-6和hs-CRPP水平与脑梗死体积大小以及神经功能缺损程度明显相关,可提示ACI患者病情的发生发展以及严重程度.

Objective To investigate the correlation between serum signal transducer and activator of transcription (STAT)-4,6,neurologic impairment and cerebral infarction volume in patients of acute cerebral infarction (ACI).Methods 122 patients with ACI and 74 synchronously examined healthy people(controls) were collected.The surum levels of STAT-4,6 and Hs-CRP were detected by eyzyme-linked immunosorbent assay (ELISA) and immunity transmission turbidity in all the subjected.The patients of cerebral infarction volume with ACI was calculated by pullicino formula,which were divided into small,middle and large infarction group.Based on the neurological functional deficit scale from standareds of CSS in 1995,the patients were divided into mild,moderate,severe impairment.Changes of the level of STAT-4,6 and Hs-CRP were compared in ACI patients with different cerebral infarction volume and different impairment degree.Linear correlation and regression analysis were performed to STAT-4,6 and Hs-CRP.Results The levels of serum STAT-4 and Hs-CRP were higher in ACI group than those in the control group [(225.82 ± 113.39),(16.86 ±4.17) ng/L vs.(107.72 ± 75.18),(6.27 ± 1.15) ng/L],however STAT-6 lower than those in the control group [(305.89 ± 117.26) ng/L vs.(421.65 ± 107.76) ng/L],and the differences was statically significant(P < 0.05).The larger the infarction volume and the severer the neurologic impairment degree was in the patients with ACI,the higher the levels of serum STAT-4 and Hs-CRP,however the lower the levels of STAT-6.Linear regression analysis results showed that the level of STAT-4 was significantly positively correlated with the level of Hs-CRP,STAT-6 negative correlation (P < 0.05).Conclusion There is a significant correlation between serm STAT-4,6 levels and infarct volume,neurologic impairment in ACI patients,which can provide some indicative effect for the occurrence and development and neurologic impairment degree of ischemic cerebrovascular diseases.
