目的:探讨超敏 C 反应蛋白(hs‐CRP),同型半胱氨酸(HCY),补体 C3、C4,血脂水平[胆固醇(TC)、三酰甘油(TG)]变化在2型糖尿病(T2DM )及其并发症中的作用。方法选择 T2DM 患者112例,根据有无并发症,分为并发症组和单纯组,其中并发症组根据并发症分为大血管病变组、合并感染组、糖尿病肾病组、视网膜病变组和周围神经病变组5个亚组。同时选择健康体检人员52例作为健康对照组。测定各组 hs‐CRP 、HCY 、C3、C4、TC 、TG 的水平,对结果进行分析。结果与健康对照组比较,T2DM 组 hs‐CRP 、HCY 、C3、C4、TC 、TG 均显著升高(P<0.05)。与单纯组比较,大血管病变组 HCY 、TC 明显升高(P<0.05),合并感染组、糖尿病肾病组、视网膜病变组、周围神经病变组 hs‐CRP 、HCY 及 TC 明显升高(P <0.05)。并发症各亚组 C3、C4水平均较单纯组显著降低(P<0.05)。合并感染组 hs‐CRP 升高最明显,大血管病变组 HCY 升高最显著。结论对 T2DM 患者进行 hs‐CRP 、HCY 、C3、C4、TC 、TG 的监测,有助于早期预防糖尿病并发症。

목적:탐토초민 C 반응단백(hs‐CRP),동형반광안산(HCY),보체 C3、C4,혈지수평[담고순(TC)、삼선감유(TG)]변화재2형당뇨병(T2DM )급기병발증중적작용。방법선택 T2DM 환자112례,근거유무병발증,분위병발증조화단순조,기중병발증조근거병발증분위대혈관병변조、합병감염조、당뇨병신병조、시망막병변조화주위신경병변조5개아조。동시선택건강체검인원52례작위건강대조조。측정각조 hs‐CRP 、HCY 、C3、C4、TC 、TG 적수평,대결과진행분석。결과여건강대조조비교,T2DM 조 hs‐CRP 、HCY 、C3、C4、TC 、TG 균현저승고(P<0.05)。여단순조비교,대혈관병변조 HCY 、TC 명현승고(P<0.05),합병감염조、당뇨병신병조、시망막병변조、주위신경병변조 hs‐CRP 、HCY 급 TC 명현승고(P <0.05)。병발증각아조 C3、C4수평균교단순조현저강저(P<0.05)。합병감염조 hs‐CRP 승고최명현,대혈관병변조 HCY 승고최현저。결론대 T2DM 환자진행 hs‐CRP 、HCY 、C3、C4、TC 、TG 적감측,유조우조기예방당뇨병병발증。
Objective To explore the role of hs‐CRP ,HCY ,C3 ,C4 and lipid levels(TC ,TG) in type 2 diabetes mellitus(T2DM ) and its complications .Methods 112 patients with T2DM were selected and divided into the compli‐cations group and simple group according to whether having complications .,in which the complications group were re‐divided into the 5 subgroups of macroangiopathy ,complicating infection ,diabetic nephropathy ,retinopathy and pe‐ripheral neuropathy .Contemporaneous 52 healthy individuals of physical examination were selected as the healthy control group .The hs‐CRP ,HCY ,C3 ,C4 ,TC and TG levels were detected in each group .The detection results were analyzed .Results The levels of hs‐CRP ,HCY ,C3 ,C4 ,TC and TG in the T2DM group were significantly increased compared with the healthy control group(P< 0 .05) .The levels of HCY and TC in the macroangiopathy group were significantly increased compared with the simple group(P< 0 .05) ,the levels of hs‐CRP ,HCY and TC in the compli‐cating infection ,diabetic nephropathy ,retinopathy and peripheral neuropath groups were significantly increased(P<0 .05) .The levels of C3 and C4 in the diabetic complications subgroups were significantly decreased compared with the simple group(P< 0 .05) .The increase of hs‐CRP level in the complicating infection group was most obvious and the increase of HCY level in the macroangiopathy group was most significant .Conclusion Monitoring hs‐CRP , HCY ,C3 ,C4 ,TC and TG in the patients with T2DM conduces to early prevent diabetic complications .
