目的:探讨术中超声辅助显微神经外科手术治疗脑深部肿瘤的临床价值。方法选择我院2009年10月至2014年7月收治的50例脑深部肿瘤患者的资料,均行显微神经外科手术治疗,术前进行颅脑核磁共振(MRI)检查,术中采用超声确定病变位置、病灶深度及与周围组织关系,行肿瘤切除术,对超声在显微神经外科手术中辅助定位脑深部肿瘤的价值进行评价。结果在本组50例患者中,胶质瘤32例,海绵状血管瘤8例,脉络膜乳头状瘤5例,脑膜瘤5例。50例患者肿瘤定位准确,瘤体完全切除,术后恢复良好。与术前MRI诊断相比较,术中超声实时瘤体体积与MRI结果基本一致,但超声有探测能力强、实时、随时探测的术中指导能力。结论颅脑深部肿瘤显微外科术中应用超声技术,具有图像实时可靠、定位准确的特点,对于提高肿瘤的全切率有积极意义。

목적:탐토술중초성보조현미신경외과수술치료뇌심부종류적림상개치。방법선택아원2009년10월지2014년7월수치적50례뇌심부종류환자적자료,균행현미신경외과수술치료,술전진행로뇌핵자공진(MRI)검사,술중채용초성학정병변위치、병조심도급여주위조직관계,행종류절제술,대초성재현미신경외과수술중보조정위뇌심부종류적개치진행평개。결과재본조50례환자중,효질류32례,해면상혈관류8례,맥락막유두상류5례,뇌막류5례。50례환자종류정위준학,류체완전절제,술후회복량호。여술전MRI진단상비교,술중초성실시류체체적여MRI결과기본일치,단초성유탐측능력강、실시、수시탐측적술중지도능력。결론로뇌심부종류현미외과술중응용초성기술,구유도상실시가고、정위준학적특점,대우제고종류적전절솔유적겁의의。
Objective To discuss the intraoperative ultrasound assisted microsurgery surgical treatment the clinical value of deep brain tumor.Methods From October 2009 to July 2014 were 50 cases of patients with deep brain tumor information, both microscopic nerve surgery, preoperative brain magnetic resonance (MRI) examination, intraoperative using ultrasonic determine lesion location, the focal depth and the relationship with the surrounding tissue, tumor resection, auxiliary positioning of ultrasound in the micro neurosurgery to evaluate the value of the deep brain tumors.Results Gliomas in 50 cases patients, 32cases, 8 cases of cavernous hemangioma,5 cases of inverted papilloma of the choroid, 5 cases with meningioma. 50 cases of patients with tumor location accurate, tumors had complete resection, postoperative recovered well. Compared with the preoperative MRI diagnosis and intraoperative ultrasound real-time tumors volume and MR results are basically identical, but there are ultrasonic detection capability, real-time, detection ability of intraoperative guidance at any time.Conclusion Deep brain tumor microsurgery technique in the application of ultrasonic technology, has the characteristics of image real-time and reliable, accurate positioning, to enhance the tumor cut rate has a positive meaning.
