目的探讨人工关节置换术术后导致手术部位感染的相关因素,以采取预防控制措施。方法选择2012年1月至2014年1月医院收治的639例初次人工关节置换手术患者作为研究对象,对术后手术部位感染发生危险因素进行多因素logistic回归分析。结果639例人工关节置换术后发生手术部位感染21例,感染率为3.20%;多因素logistic回归分析结果表明年龄[OR95% CI:1.319(1.146-1.701)]、糖尿病[OR95% CI:2.973(1.667-5.309)]、肥胖[OR95% CI:1.213(1.021-1.659)]、麻醉评分[OR95% CI:2.399(2.186-2.854)]、低蛋白血症[OR95% CI:3.213(1.751-5.895)]、低血红蛋白[OR95% CI:3.024(1.741-5.256)]、手术持续时间[OR95% CI:2.973(1.372-4.301)]是人工关节置换术术后手术部位感染的危险因素。结论年龄、糖尿病、肥胖、麻醉评分、低蛋白血症、低血红蛋白、手术持续时间均会增加人工关节置换术术后手术部位感染的风险。

Objective To study the related factors of surgical site infection after artificial joint replacement in order to take corresponding prevention and control measures.Methods In total of 639 patients received artificial joint replacement from January 2012 to January 2014 were included in our study. Multiple logistic regression analysis was conducted to analyze the risk factors of postoperative surgical site infection.ResultsAmong the 639 cases, 21 patients had surgical site infections and the infection rate was 3.20%; According to the results of multivariate logistic regression analysis, age[OR95% CI:1.319(1.146-1.701)], diabetes[OR95% CI:2.973(1.667-5.309)], obesity[OR95% CI:1.213(1.021-1.659)], anesthesia score[OR95% CI:2.399(2.186-2.854)], hypoalbuminemia[OR95% CI:3.213(1.751-5.895)], low hemoglobin[OR95% CI:3.024(1.741-5.256)]and duration time of surgery[OR95% CI:2.973(1.372-4.301)]were the risk factors of surgical site infections.Conclusion Risk factors including age, diabetes, obesity, anesthesia score, hypoalbuminemia, low hemoglobin and duration time of surgery can increase the risk of surgical site infection after artificial joint replacement.
