目的:探讨急性呼吸窘迫综合征(ARDS)患者应用肺复张法的临床疗效。方法选取2010年1月~2011年12月我院ICU收治的27例ARDS患者为研究对象,对其应用肺复张法治疗,观察并记录患者肺复张前和肺复张后4 h的呼吸力学指标、氧合指数等指标。结果27例ARDS肺复张法治疗患者中,23例存活,死亡的有4例患者;患者肺复张法治疗后PaO2/FiO2高于肺复张法治疗前(P<0.05),肺顺应性、SaO2较肺复张法治疗前增加(P<0.05);患者PaCO2肺复张法治疗前后差异不显著。结论 ARD患者应用肺复张法可改善氧合和肺顺应性,是肺保护性通气策略的重要补充。

목적:탐토급성호흡군박종합정(ARDS)환자응용폐복장법적림상료효。방법선취2010년1월~2011년12월아원ICU수치적27례ARDS환자위연구대상,대기응용폐복장법치료,관찰병기록환자폐복장전화폐복장후4 h적호흡역학지표、양합지수등지표。결과27례ARDS폐복장법치료환자중,23례존활,사망적유4례환자;환자폐복장법치료후PaO2/FiO2고우폐복장법치료전(P<0.05),폐순응성、SaO2교폐복장법치료전증가(P<0.05);환자PaCO2폐복장법치료전후차이불현저。결론 ARD환자응용폐복장법가개선양합화폐순응성,시폐보호성통기책략적중요보충。
Objective To investigate the clinical effect of the application of pulmonary rehabilitation in patients with acute respiratory distress syndrome(ARDS). Methods 27 cases of ARDS patients from January 2010 to December 2011 in our hospital admitted to the intensive care unit were selected as the research object,who were applicated pulmonary rehabilitation metho,and the indexes of respiratory mechanics and oxygenation of patients before and after 4 h of pulmonary rehabilitation were observed and recorded.ResultsIn 27 cases of ARDS by pulmonary rehabilitation treatment,there were 23 cases survival, 4 patients dead.The oxygenation index (PaO2/FiO2) was higher than that before pulmonary rehabilitation(P< 0.05).Lung compliance and percutaneous oxygen saturation (SaO2) were also higher than those before pulmonary rehabilitation(P< 0.05).Carbon dioxide partial pressure (PaCO2)index was not significantly changed compared with before pulmonary rehabilitation.ConclusionThe application of pulmonary rehabilitation in patients with ARDS can improve oxygenation and lung compliance,and it is an important supplement to the lung protective ventilation strategy.
