Acute coronary syndrome%microRNA-155%IL-17%NT-BNP
目的:探讨microRNA-155(miR-155)在急性冠状动脉综合征(acute coronary syndrome, ACS)中的表达水平及临床意义。方法选取2013年1月至2014年1月本院急诊医学科及心血管内科确诊的ACS患者62例为研究对象,其中,32例急性心肌梗死患者纳入观察组,30例不稳定型心绞痛患者纳入对照组。选取本院同期健康体检者20例纳入正常组。取各组研究对象外周血,分离获取单核细胞。采用Trizol RNA提取方法和microRNA快速提取试剂盒分别提取单核细胞及血浆RNA,采用逆转录-聚合酶链式反应(RT-PCR)及实时定量逆转录-聚合酶链式反应(qRT-PCR)检测miR-155表达水平,比较各组间miR-155的表达差异;同时结合各组研究对象血浆白细胞介素-17(IL-17)、B型利钠肽(NT-BNP)水平及冠状动脉狭窄程度等临床资料探讨miR-155表达水平的临床意义。结果观察组和对照组患者发病后2、6、12、24小时血浆及单核细胞中miR-155表达水平均显著低于正常组,血浆IL-17、NT-BNP水平均显著高于正常组(P<0.05);观察组患者冠状动脉狭窄程度重于对照组(P<0.05)。结论 ACS患者血浆及单核细胞中miR-155表达水平较健康人群均显著降低,同时血浆IL-17、NT-BNP水平均显著高于健康人群,提示miR-155通过参与炎性反应在ACS的发生中发挥重要作用。

목적:탐토microRNA-155(miR-155)재급성관상동맥종합정(acute coronary syndrome, ACS)중적표체수평급림상의의。방법선취2013년1월지2014년1월본원급진의학과급심혈관내과학진적ACS환자62례위연구대상,기중,32례급성심기경사환자납입관찰조,30례불은정형심교통환자납입대조조。선취본원동기건강체검자20례납입정상조。취각조연구대상외주혈,분리획취단핵세포。채용Trizol RNA제취방법화microRNA쾌속제취시제합분별제취단핵세포급혈장RNA,채용역전록-취합매련식반응(RT-PCR)급실시정량역전록-취합매련식반응(qRT-PCR)검측miR-155표체수평,비교각조간miR-155적표체차이;동시결합각조연구대상혈장백세포개소-17(IL-17)、B형리납태(NT-BNP)수평급관상동맥협착정도등림상자료탐토miR-155표체수평적림상의의。결과관찰조화대조조환자발병후2、6、12、24소시혈장급단핵세포중miR-155표체수평균현저저우정상조,혈장IL-17、NT-BNP수평균현저고우정상조(P<0.05);관찰조환자관상동맥협착정도중우대조조(P<0.05)。결론 ACS환자혈장급단핵세포중miR-155표체수평교건강인군균현저강저,동시혈장IL-17、NT-BNP수평균현저고우건강인군,제시miR-155통과삼여염성반응재ACS적발생중발휘중요작용。
ObjectiveTo investigate the expression levels and clinical signiifcance of microRNA-155 (miR-155) in patients with acute coronary syndrome (ACS).Method62 patients with ACS (32 patients with AMI in observation group, 30 patients with UA in control group) from January 2013 to January 2014 in department of emergency and cardiovascular in our hospital were selected as objects of study. 20 healthy cases were selected as normal group. The study subjects were isolated from peripheral blood, mononuclear cells were isolated from each group. By Trizol RNA method and microRNA rapid extraction kit, RNA in monocytes and plasma were extracted, used RT-PCR and qRT-PCR to detect expression levels of miR-155 among the three groups; plasma IL-17 and NT-BNP levels and coronary artery stenosis and other clinical trial data were analyzed to discuss clinical significance of miR-155.ResultPlasma and monocytes miR-155 expression levels in observation group and control group after the onset of 2, 6, 12 and 24 hours were signiifcantly lower than normal group. The plasma IL-17 and NT-BNP levels in observation group and control group were signiifcantly higher than normal group (P<0.05). Coronary stenosis degree in observation group was more severe than control group (P<0.05). ConclusionExpression level of miR-155 in plasma and monocytes of ACS patients are signiifcantly reduced than normal healthy population, while plasma IL-17, NT-BNP levels are significantly higher than normal healthy population, it shows that miR-155 plays an important role in the occurrence of ACS.
