目的：分析非小细胞肺癌患者外周血与癌组织Her－2基因突变的一致性，探讨检测外周血Her－2基因突变的临床应用价值。方法应用实时荧光定量PCR方法检测185例非小细胞肺癌患者外周血标本和癌组织标本Her－2基因突变情况，比较外周血和癌组织标本Her－2基因突变一致性，分析Her－2基因突变和患者临床特征的相关性。结果组织标本中检测出7例突变（3．78％），外周血中4例突变（2．16％），两种标本一致者占75．00％（Kappa＝0．553，P＜0．05）。外周血Her－2基因突变发生率与吸烟史有关（P＜0．05），而与年龄、性别和病理类型无关系（P＞0．05）。癌组织Her－2基因突变发生率与性别、吸烟史有关（P＜0．05），而与年龄、病理类型无关（P＞0．05）。结论非小细胞肺癌患者外周血与组织Her－2基因突变一致性一般，但在无法获得癌组织标本时可以检测外周血Her－2基因状态，也可为非小细胞肺癌的临床诊疗提供一定的参考。

Objective To analyze the consistency of human epidermal growth factor receptor 2( Her-2) gene mutation in peripheral blood and tumor tissue of patients with non-small cell lung cancer,and to discuss the clinical value of Her-2 gene mutation in peripheral blood.Methods Real-time fluorescent quantitative PCR was used to determine the Her-2 gene mutation of peripheral blood and tumor tissue specimens collected from 185 cases of non-small cell lung cancer.A comparison of Her-2 gene mutation consistency of peripheral blood and tumor tissue specimens was conducted.The correlation between Her-2 gene mutations and clinical characteristics of the patients was analyzed. Results Seven(3.78%) mutation cases were observed in tissue specimens,4(2.16%) in peripheral blood specimens,the mutation consistency was 75.00%in peripheral blood-tumor tissue matched specimens(Kappa=0.553,P<0.05).The incidence of Her-2 gene mutation in peripheral blood correlate with smoking history(P<0.05),but did not correlate with age,gender and pathological type(P>0.05).The incidence of Her-2 gene mutation in tumor tissue correlated with gender and smoking history(P<0.05),but did not correlate with age and pathological type. Conclusion The consistency of Her-2 gene mutation in peripheral blood and tissue of patients with non-small cell lung cancer is common,which can be used to determine the feature of Her-2 gene mutation in peripheral blood when the tissue specimen is not available. It might provide some references for the diagnosis and treatment in non-small cell lung cancer.
