目的:探讨在冠状动脉CTA检查中,根据小剂量实验对比剂峰值的高低,对冠脉成像注射速率进行针对性调整的方法和意义。方法260例患者随机分为两组(A组130例,B组130例)。所有病例检查均采用小剂量对比剂团注实验法。 A组冠脉增强扫描对比剂注射速率与小剂量实验保持一致,B组则根据小剂量实验中主动脉根部CT峰值情况进行相应调整。对两组冠脉图像质量进行分析、记录及统计学比较。结果 B组冠脉图像质量Ⅰ级98例,较A组多32例;Ⅱ级29例,较A组少20例;Ⅲ级3例,较A组少12例,两组间差异有统计学意义。结论根据小剂量实验对比剂峰值的高低,对下一步冠脉增强对比剂注射速率进行弥补性调整,能明显提高CCTA的检查成功率,在很大程度上改善冠脉图像质量。

목적:탐토재관상동맥CTA검사중,근거소제량실험대비제봉치적고저,대관맥성상주사속솔진행침대성조정적방법화의의。방법260례환자수궤분위량조(A조130례,B조130례)。소유병례검사균채용소제량대비제단주실험법。 A조관맥증강소묘대비제주사속솔여소제량실험보지일치,B조칙근거소제량실험중주동맥근부CT봉치정황진행상응조정。대량조관맥도상질량진행분석、기록급통계학비교。결과 B조관맥도상질량Ⅰ급98례,교A조다32례;Ⅱ급29례,교A조소20례;Ⅲ급3례,교A조소12례,량조간차이유통계학의의。결론근거소제량실험대비제봉치적고저,대하일보관맥증강대비제주사속솔진행미보성조정,능명현제고CCTA적검사성공솔,재흔대정도상개선관맥도상질량。
Objective To study the method and value of the remedial adjustment of injection rate in the course of CT coronary angiography. Methods 260 patients were randomly divided into 2 groups with 130 patients in each group. All cases were per-formed with “Timing bolus”. The injection rate of cases in group A was identical with the“Timing bolus”. The injection rate of cas-es in group B was adjusted according to the leve of peak value of contrast concentration in blood vessel at aortic root. Image quali-ty of the coronary in two groups were analysed and enregistered,and then were made a statistical comparison. Results 98 first quality images in group B,32 more than in group A. 29 second quality images in group B,20 less than in group A. 3 third quality images in group B,12 less than in group A. There were significant statistical difference between two groups. Conclusion Accord-ing to the leve of peak value of contrast concentration in blood vessel at aortic root to adjust the injection rate in the course of CT coronary angiography,which is contribute to improve the success rate of CCTA and to improve image quality of coronary.
