目的：观察尼可地尔对非血运重建的冠脉严重三支病变患者的疗效。方法选择冠状动脉造影证实冠脉严重三支病变的冠心病患者72例，随机分为尼可地尔治疗组（观察组 n＝35）和常规治疗组（对照组 n＝37），对照组患者视病情给予长效硝酸脂类、拜阿司匹林、氯吡格雷、他汀类、ACEI、钙离子拮抗剂等常规药物治疗；观察组在此基础上加用尼可地尔治疗。对比二组患者治疗前后心绞痛缓解情况及6 min步行实验改善情况。结果两组患者在治疗后心绞痛发作较前均有缓解，6 min步行实验距离较前提高，而尼可地尔治疗组更为明显，差异有统计学意义。结论在常规冠心病二级预防药物基础上加用尼可地尔对非血运重建的冠脉严重三支病变患者可显著改善心绞痛发作及提高活动耐量。

목적：관찰니가지이대비혈운중건적관맥엄중삼지병변환자적료효。방법선택관상동맥조영증실관맥엄중삼지병변적관심병환자72례，수궤분위니가지이치료조（관찰조 n＝35）화상규치료조（대조조 n＝37），대조조환자시병정급여장효초산지류、배아사필림、록필격뢰、타정류、ACEI、개리자길항제등상규약물치료；관찰조재차기출상가용니가지이치료。대비이조환자치료전후심교통완해정황급6 min보행실험개선정황。결과량조환자재치료후심교통발작교전균유완해，6 min보행실험거리교전제고，이니가지이치료조경위명현，차이유통계학의의。결론재상규관심병이급예방약물기출상가용니가지이대비혈운중건적관맥엄중삼지병변환자가현저개선심교통발작급제고활동내량。
Objective Efficacy of nicorandil on non -coronary revascularization in patients with severe three-vessel disease .Methods 7 2 patients with Coronary angiography showed severe three branch disease was selected .The patients were randomly divided into nicorandil treatment group (observation group ,n=3 5 ) and conventional treatment group (control group ,n= 3 7 ) ,depending on the condition of patients in the control group given aspirin ,clopidogrel ,statins class ,ACEI ,calcium antagonists ,nitric acid and other conventional therapy ;observation group was based on the use of nicorandil treatment .Comparing two groups of patients be-fore and after treatment of angina and the 6 -minute walk test cases to improve the situation .Results Two groups of patients after treatment were compared with the previous relieve angina pectoris ,6 minutes walking distance from the previous experiment to improve ,further nicorandil treatment group is more obvious ,the dif-ference was statistically significant .Conclusion For non -coronary revascularization in patients with severe three-vessel disease ,in the conventional secondary prevention of coronary heart disease drugs based on the use of nicorandil curative efforts for nicorandil is obviously superior to improved angina and improve exercise toler -ance .
