目的:探讨子宫孕晚期妊娠压力对初产妇母儿免疫功能的影响。方法:分娩前1 d,采用妊娠压力量表对我院产科门诊就诊的140例初产妇进行问卷调查,评定妊娠压力水平。分娩过程中抽取脐静脉血,并于分娩后24 h 内采集产妇静脉血,采用免疫比浊法测定免疫球蛋白 IgA、IgG、IgM 以及补体 C3、C4含量。结果:初产妇分娩前1 d 妊娠压力得分中位数(M)为1.53、四分位间距(QR)为0.49,引起初产妇妊娠压力的主要来源是“确保母子健康和安全而引发的压力感”。妊娠压力评分与免疫因子 IgA、C4无相关性(P >0.05),与免疫因子 IgG、IgM、C3呈负相关(P <0.05)。结论:初产妇在分娩前1 d 压力水平处在中度压力水平,产妇妊娠压力越小,母儿的免疫功能越好,反之亦然。

목적:탐토자궁잉만기임신압력대초산부모인면역공능적영향。방법:분면전1 d,채용임신압역량표대아원산과문진취진적140례초산부진행문권조사,평정임신압력수평。분면과정중추취제정맥혈,병우분면후24 h 내채집산부정맥혈,채용면역비탁법측정면역구단백 IgA、IgG、IgM 이급보체 C3、C4함량。결과:초산부분면전1 d 임신압력득분중위수(M)위1.53、사분위간거(QR)위0.49,인기초산부임신압력적주요래원시“학보모자건강화안전이인발적압력감”。임신압력평분여면역인자 IgA、C4무상관성(P >0.05),여면역인자 IgG、IgM、C3정부상관(P <0.05)。결론:초산부재분면전1 d 압력수평처재중도압력수평,산부임신압력월소,모인적면역공능월호,반지역연。
Objective:To discuss the influence of stress in late pregnancy on the immune function of the puerperant and infants. Meth-ods:140 primiparas in our hospital were investigated by using pregnancy stress scale before one day of delivery and the stress level was as-sessed. IgA,IgG,IgM and the content of C3 and C4 in serum were determined during delivery and within 24 hours after delivery. Results:The scores of stress(M)were 1. 53 before one day of delivery and QR was 0. 49. There was no correlation between the scores of pregnancy stress and IgA and C4(P > 0. 05);the scores of pregnancy stress were negatively correlated with IgG,C3 and IgM(P < 0. 05). Conclu-sion:The pregnancy stress of primiparas is in medium level before one day of delivery,the milder the stress of primiparas is,the better the immune function of puerperant and infants is.
