目的：探讨来源于呼吸道肺炎克雷伯菌(KP)耐药性与CTX-M型超广谱β-内酰胺酶(ESBLs)的关系。方法采用PCR 扩增和测序分析检测 KP 质粒和染色体上的 CTX-M 型 ESBLs 基因；采用纸片扩散法检测抗菌药物敏感性；使用χ2检验分析相关数据。结果有 CTX-M-1群酶和 CTX-M-25群酶检出,CTX-M 型 ESBLs 的总检出率为39.02%(48株),其中质粒和染色体上同时携带 CTX-M-1群酶的检出率为26.02%(32株)。β-内酰胺类抗菌药物头孢唑林、氟喹诺酮类抗菌药物左旋氧氟沙星、磺胺类抗菌药物复方磺胺甲恶唑等在 CTX-M 型 ESBLs 阳性组的耐药率明显高于阴性组(P <0.05)。结论在来源于呼吸道的 KP 中,在质粒和染色体上同时携带 CTX-M-1群酶基因最常见,CTX-M 型 ESBLs 与 KP 对某些抗菌药物产生耐药性关系密切。
목적：탐토래원우호흡도폐염극뢰백균(KP)내약성여CTX-M형초엄보β-내선알매(ESBLs)적관계。방법채용PCR 확증화측서분석검측 KP 질립화염색체상적 CTX-M 형 ESBLs 기인；채용지편확산법검측항균약물민감성；사용χ2검험분석상관수거。결과유 CTX-M-1군매화 CTX-M-25군매검출,CTX-M 형 ESBLs 적총검출솔위39.02%(48주),기중질립화염색체상동시휴대 CTX-M-1군매적검출솔위26.02%(32주)。β-내선알류항균약물두포서림、불규낙동류항균약물좌선양불사성、광알류항균약물복방광알갑악서등재 CTX-M 형 ESBLs 양성조적내약솔명현고우음성조(P <0.05)。결론재래원우호흡도적 KP 중,재질립화염색체상동시휴대 CTX-M-1군매기인최상견,CTX-M 형 ESBLs 여 KP 대모사항균약물산생내약성관계밀절。
Objective To investigate the relationship between the resistance of klebsiella pneumoniae(KP)which come from respiratory tract and CTX-M extended spectrumβ-lactamase(ESBLs).Methods Detecting CTX-M ESBLs genes in the plasmid and chromosomes were used by PCR and sequence analysis;Disk diffusion method was sued by antibiotics susceptibility test;Chi square test was sued by analytic the data.Results CTX-M-1 ESBLs and CTX-M-25 ESBLs were detected in the KP and the total detection rate of CTX-M ESBLs was 39.02%(48 strains),the detection rate of CTX-M-1 ESBLs was 26.02%(32 strains)which existence in the plasmid and chromosomes simultaneously.The resistance rate of β-lactam antibiotic cefazolin,fluoroquinolone antibiotic levo-floxacin and sulfonamides antibiotic bactrim and so on in positive group of CTX-M ESBLs were significantly higher than negative group of CTX-M ESBLs(P <0.05).Conclusion CTX-M-1 ESBLs gene which concurrent in the plasmid and chromosomes may the most common.KP resistance to certain antibiotics closely is relate to the CTX-M ESBLs.
