目的:探讨胰岛素样生长因子1受体(IGF‐1R)和血管内皮生长因子(VEGF)在肝细胞癌(HCC)中的表达与HCC临床病理特征之间的关系及在HCC预后评估中的意义。方法采用免疫组织化学EnVision两步法检测石蜡包埋 HCC病例癌组织及配对癌旁组织中IGF‐1R及VEGF的蛋白表达,并计数微血管密度(MVD),结合临床病理特征和随访结果进行分析。结果(1) IGF‐1R、VEGF的蛋白表达水平及MVD计数在癌组织高于癌旁组织(P<0.05);(2)IGF‐1R及VEGF的蛋白表达在不同病理分级、TNM 分期、有无血管浸润及预后组间比较差异均有统计学意义(P<0.05)。(3)IGF‐1R及VEGF的蛋白表达强度与MVD水平、IGF‐1R的蛋白表达强度与VEGF的蛋白表达强度间均呈正相关。结论 IGF‐1R及VEGF促进了 HCC的发生和发展且与预后密切相关,可用于其预后评估。

목적:탐토이도소양생장인자1수체(IGF‐1R)화혈관내피생장인자(VEGF)재간세포암(HCC)중적표체여HCC림상병리특정지간적관계급재HCC예후평고중적의의。방법채용면역조직화학EnVision량보법검측석사포매 HCC병례암조직급배대암방조직중IGF‐1R급VEGF적단백표체,병계수미혈관밀도(MVD),결합림상병리특정화수방결과진행분석。결과(1) IGF‐1R、VEGF적단백표체수평급MVD계수재암조직고우암방조직(P<0.05);(2)IGF‐1R급VEGF적단백표체재불동병리분급、TNM 분기、유무혈관침윤급예후조간비교차이균유통계학의의(P<0.05)。(3)IGF‐1R급VEGF적단백표체강도여MVD수평、IGF‐1R적단백표체강도여VEGF적단백표체강도간균정정상관。결론 IGF‐1R급VEGF촉진료 HCC적발생화발전차여예후밀절상관,가용우기예후평고。
Objective To investigate the expression of insulin‐like growth factor 1 receptor (IGF‐1R) ,vascular endothelial growth factor (VEGF) and their relationships between clinic opathologic features in hepatocellular carci‐noma (HCC) .Method Immunohistochemical envision method was utilized to detect the expression of IGF‐1R and VEGF ,MVD was counted .The follow‐up data of the cases of HCC was analyzed .Result The IGF‐1R and VEGF protein shown a higher expression in HCC than in the tumor‐adjacent tissues (P< 0 .05) .The difference of the number of IGF‐1R and VEGF protein overexpression in histological grade and TNM stage and prognosis was signifi‐cant (P<0 .05) ,moreover ,the vascular infiltration group was significantly more than group without vascular inva‐sion (P<0 .05) .The value of MVD was higher in HCC than that in tumor‐adjacent tissues .By Spearman rank cor‐relation analysis ,the level of MVD was positively correlated with IGF‐1R protein expression and VEGF protein ex‐pression .Moreover ,the expression of IGF‐1R and VEGF protein was also positive correlation .Conclusion IGF‐1R and VEGF protein may promote the occurrence and development of HCC ,and they all promote the neovasculation in HCC .The expression of IGF‐1R and VEGF can be used as a marker to evaluate the prognosis of HCC .
