目的：探讨“医护一体化”模式在降低糖尿病低血糖患者发生率中的应用效果。方法：对1058例住院糖尿病患者进行了24651次末梢血糖监测，将2013年3~5月581例共12192次末梢血糖监测记录（干预前）和2013年6~8月477例12459次末梢血糖监测记录（干预后）的低血糖发生率进行统计分析，分析干预前低血糖发生原因，观察采用“医护一体化”模式进行血糖管理的效果。结果：按末梢血糖监测时间段进行干预前后比较，干预后低血糖的总体发生率由6.56‰下降至2.32‰（P ﹤0.01），空腹低血糖发生率由1.31‰下降至0.48‰（P ﹤0.05）、餐后低血糖发生率由3.36‰下降至1.04‰（P ﹤0.01）、夜间低血糖发生率0.57‰下降至0.08‰（P ﹤0.01）。结论：将“医护一体化”模式应用于糖尿病患者血糖管理中，让医生、护士、患者三位一体紧密配合，不仅能有效降低低血糖的发生率及其相关风险，还能减轻患者痛苦，延缓糖尿病并发症的出现，提高医疗护理质量及患者满意度。
목적：탐토“의호일체화”모식재강저당뇨병저혈당환자발생솔중적응용효과。방법：대1058례주원당뇨병환자진행료24651차말소혈당감측，장2013년3~5월581례공12192차말소혈당감측기록（간예전）화2013년6~8월477례12459차말소혈당감측기록（간예후）적저혈당발생솔진행통계분석，분석간예전저혈당발생원인，관찰채용“의호일체화”모식진행혈당관리적효과。결과：안말소혈당감측시간단진행간예전후비교，간예후저혈당적총체발생솔유6.56‰하강지2.32‰（P ﹤0.01），공복저혈당발생솔유1.31‰하강지0.48‰（P ﹤0.05）、찬후저혈당발생솔유3.36‰하강지1.04‰（P ﹤0.01）、야간저혈당발생솔0.57‰하강지0.08‰（P ﹤0.01）。결론：장“의호일체화”모식응용우당뇨병환자혈당관리중，양의생、호사、환자삼위일체긴밀배합，불부능유효강저저혈당적발생솔급기상관풍험，환능감경환자통고，연완당뇨병병발증적출현，제고의료호리질량급환자만의도。
Objective:To explore the effect of applying nurse - physician collaboration model in reducing the incidence of hypoglycemia in diabetic patients. Methods:The blood sugar monitoring was altogether conducted 24651 times in 1058 patients. The incidence of hypogly-cemia in diabetic patients was statistically analyzed according to the monitoring of 12192 times in 581 patients before the intervention (March to May 2013)and the monitoring of 12459 times in 477 patients after the intervention(June to August 2013). The effect of apply-ing nurse - physician collaboration model on blood sugar control was observed. Results:After the intervention,the incidence of hypoglyce-mia decreased from 6. 56‰ to 2. 32‰(P ﹤ 0. 01);fasting hypoglycemia fell from 1. 31‰ to 0. 48‰(P ﹤ 0. 05);postprandial hypoglyce-mia dropped from 3. 36 to 1. 04%(P ﹤ 0. 01)and the incidence of nocturnal hypoglycemia reduced from 0. 57‰ to 0. 08‰(P ﹤ 0. 01). Conclusion:Application of nurse - physician collaboration model can not only effectively reduce the incidence of hypoglycemia and related risks,but also relive the suffering of the patients and delay the emergence of diabetic complications so as to improve the quality of medical care and patient satisfaction.
