目的:探讨颈动脉支架成形术(carotid artery stenting,CAS)治疗症状性颈动脉狭窄的短期疗效和安全性. 方法:采用 CAS治疗43例症状性颈内动脉狭窄患者,于围手术期给予抗血小板及他汀类药物治疗. 结果:43 例患者全部成功植入颈内动脉支架,颈内动脉管腔狭窄率由术前的(84.25 ±10.67) %下降至(7.81 ±3.47) %,两者比较差异有统计学意义( P <0.01);术中有5例发生一过性心率减慢、血压降低,经积极治疗后好转;术后1例发生脑梗死,经治疗后无神经系统后遗症,其余患者术后均未发生缺血性事件,随访3~12个月颈动脉彩超均未显示再狭窄. 结论:CAS治疗症状性颈动脉狭窄安全有效,短期疗效肯定,值得临床推广应用.

Objective:To discuss the short-term efficacy and safety of carotid artery stenting in the treatment of symptomatic carotid stenosis . Methods:43 cases of symptomatic carotid stenosis received carotid artery stenting (CAS), with antiplatelet and statin given during perioperative period.Results: All 43 patients were successfully implanted carotid artery stent.The luminal stenosis rate of carotid artery decreased from (84.25 ±10.67) % before operation to (7.81 ±3.47) %, and the difference was statistically significant ( P <0.01).There were five patients who suffered from transient slowing of heart rate and decreased blood pressure and improved after active treatment .Cerebral infarction occurred in one patient after surgery, but no sequelae of nervous system appeared after treatment.None of the rest of the patients underwent postoperative ischemic events.After 3-12 months of follow-up, carotid ultrasonography showed no restenosis.Conclusion:CAS is safe and effective for the treatment of symptomatic carotid stenosis, with short-term curative effect affirmation, which is well worthy to popularize.
