目的：观察改良腭咽成形术和悬雍垂腭咽成形术治疗阻塞性睡眠呼吸暂停综合征（ OSAS）的疗效，及其对白细胞介素－6（ IL－6）、瘦素和尿酸水平的影响。方法选择OSAS患者90例，按照手术方式分为观察组和对照组各45例，对照组予以悬雍垂腭咽成形术治疗，观察组予以改良腭咽成形术治疗，观察2组的疗效及其对鼾声分级、嗜睡评分以及IL－6、瘦素和尿酸水平的影响。结果治疗后观察组总有效率明显高于对照组（P＜0．05）。2组鼾声分级、嗜睡评分以及IL－6、瘦素和尿酸水平均较治疗前明显降低（P均＜0．05），而观察组的降低水平更为明显（P均＜0．05）。结论改良腭咽成形术治疗OSAS的疗效确切，能保留悬雍垂，维持了咽腔的基本结构，对机体内环境紊乱和炎症状态具有缓解作用。
목적：관찰개량악인성형술화현옹수악인성형술치료조새성수면호흡잠정종합정（ OSAS）적료효，급기대백세포개소－6（ IL－6）、수소화뇨산수평적영향。방법선택OSAS환자90례，안조수술방식분위관찰조화대조조각45례，대조조여이현옹수악인성형술치료，관찰조여이개량악인성형술치료，관찰2조적료효급기대한성분급、기수평분이급IL－6、수소화뇨산수평적영향。결과치료후관찰조총유효솔명현고우대조조（P＜0．05）。2조한성분급、기수평분이급IL－6、수소화뇨산수평균교치료전명현강저（P균＜0．05），이관찰조적강저수평경위명현（P균＜0．05）。결론개량악인성형술치료OSAS적료효학절，능보류현옹수，유지료인강적기본결구，대궤체내배경문란화염증상태구유완해작용。
Objective It is to observe the efficacy of modified palatopharyngoplasty and uvulopalatopharyngoplasty in pa-tients with obstructive sleep apnea syndrome (OSAS) and their influence on the levels of interleukin -6 (IL-6), leptin and uric acid.Methods 90 patients with OSAS were selected and divided into observation group and control group according to the surgical procedure, 45 cases in each group.The patients were received uvulopalatopharyngoplasty in the control group and modified palatopharyngoplast in the observation group.After treatment, the efficacy and classification snoring, sleepiness clas-sification, levels of IL-6, leptin and uric acid were observed in each groups.Results After treatment, the total effective rate in observation group was significantly better than that in control group (P<0.05);the graded snoring, sleepiness classifica-tion, and the levels of IL-6, leptin and uric acid in two groups were markedly decreased than before treatment ( all P<0.05), and the observation group was more obvious (all P<0.05).Conclusion The therapeutic effect is certain with modi-fied palatopharyngoplasty, it can retain the uvula, maintain the basic structure of the pharynx, so as to relieve the internal en-vironment disorders and inflammation.
