目的：了解某院使用抗菌药物的具体情况，提高临床合理使用抗菌药物的水平。方法随机抽查2014年第3季度180份住院部归档病历，对部分不合理使用抗菌药物的事例进行叙述和分析。结果抗菌药物使用率为67.36%；病原学送检率低，大部分凭经验选药；无感染指征的预防用药，预防用药的起点偏高，用量较大，时间过长药物更换不及时；术前过早给手术患者用药及长时间术后预防用药。结论抗菌药物使用较为广泛，但缺少药敏实验依据，有待进一步的改善。

목적：료해모원사용항균약물적구체정황，제고림상합리사용항균약물적수평。방법수궤추사2014년제3계도180빈주원부귀당병력，대부분불합리사용항균약물적사례진행서술화분석。결과항균약물사용솔위67.36%；병원학송검솔저，대부분빙경험선약；무감염지정적예방용약，예방용약적기점편고，용량교대，시간과장약물경환불급시；술전과조급수술환자용약급장시간술후예방용약。결론항균약물사용교위엄범，단결소약민실험의거，유대진일보적개선。
Objective To understand the application of antibiotics in our hospital's and improve the level of clinical rational application of antibiotics. Methods Snap check 180 archive records of 3rd quarter, and analysis the examples of irrational use of antibiotics. Results Antimicrobial agents usage was 67.36%; Low pathogenic submission rate, and most medicine ap-plied based on experience. There was premature delivery before operation and prolonged prophylaxis after operation. There were many problems including higher starting dosage, larger amount, longer time drug application and replacement is not timely. Conclusions Antimicrobial drug application was more widespread, while lacking the susceptibility test basis during the process. And it need to be further improved.
