目的:探讨亚临床甲状腺功能减低患者血脂、同型半胱氨酸及血尿酸水平变化及其可能原因。方法收集体检甲状腺功能正常患者(A 组),甲减患者(B 组)和亚临床甲状腺功能减低患者(C 组)各35例,同时测定各组胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、脂蛋白 a(LPa)、同型半胱氨酸(Hcy)及血尿酸(UA)水平。结果①与 A 组比较,B 组与 C 组促甲状腺激素(TSH)水平明显升高(P <0.05),B组 TSH 水平明显高于 C 组(P <0.05);②与 A 组比较,B 组与 C 组患者的 TG、TC、LDL-C,LPa、Hcy、UA 水平升高(P <0.05),HDL-C 水平降低;与 C 组比较,B 组患者 TG、TC、LDL、LPa 与 Hcy 水平更高,HDL-C 更低(P <0.05);③TSH 与 TC、TG、LDL-C、LPa、Hcy 及 UA 均呈正相关(r 值分别为0.71、0.39、0.50、0.58、0.4、0.35,均 P <0.05)。结论亚临床甲状腺功能减退患者的血脂异常、HCY、UA 改变的原因可能与 TSH 的水平及其功能有关系。LPa 与Hcy 水平升高是甲减患者易患心血管疾病的危险因素,因此检测血脂、LPa 与 Hcy 对预防甲减患者心血管疾病具有重要的意义。

목적:탐토아림상갑상선공능감저환자혈지、동형반광안산급혈뇨산수평변화급기가능원인。방법수집체검갑상선공능정상환자(A 조),갑감환자(B 조)화아림상갑상선공능감저환자(C 조)각35례,동시측정각조담고순(TC)、감유삼지(TG)、저밀도지단백담고순(LDL-C)、고밀도지단백담고순(HDL-C)、지단백 a(LPa)、동형반광안산(Hcy)급혈뇨산(UA)수평。결과①여 A 조비교,B 조여 C 조촉갑상선격소(TSH)수평명현승고(P <0.05),B조 TSH 수평명현고우 C 조(P <0.05);②여 A 조비교,B 조여 C 조환자적 TG、TC、LDL-C,LPa、Hcy、UA 수평승고(P <0.05),HDL-C 수평강저;여 C 조비교,B 조환자 TG、TC、LDL、LPa 여 Hcy 수평경고,HDL-C 경저(P <0.05);③TSH 여 TC、TG、LDL-C、LPa、Hcy 급 UA 균정정상관(r 치분별위0.71、0.39、0.50、0.58、0.4、0.35,균 P <0.05)。결론아림상갑상선공능감퇴환자적혈지이상、HCY、UA 개변적원인가능여 TSH 적수평급기공능유관계。LPa 여Hcy 수평승고시갑감환자역환심혈관질병적위험인소,인차검측혈지、LPa 여 Hcy 대예방갑감환자심혈관질병구유중요적의의。
Objective To investigate the changing and reason of blood lipid、homocysteine and Uric acid among pa-tients with subclinical hypothyroidism.Methods Thirty-five patients with normal thyroid function (group A),thirty-five patients with hypothyroidism (group B)and thirty-five patients with subclinical hypothyroidism (group C)were collected respectively.The levels of the total cholesterol (TC),triglyceride (TG),low density lipoprotein-cholesterol (LDL-C),high density lipoprotein-cholesterol (HDL-C),lipid protein a (LPa)、Homocysteine (Hcy)and Uric acid (UA)were determinated at the same time.Results ① Compared with group A,the level of TSH was significantly in-creased in group B and C (P <0.05),group B was significantly higher than group C (P <0.05);② Compared with group A,the level of TG,TC,LDL-C,LPa,Hcy and UA were increased in group B and C (P <0.05),while HDL-C was decreased;Compared with group C,the level of TG,TC,LDL-C,LPa,Hcy and UA in group B were higher than group C;while HDL-C in group B was lower than group C (P <0.05);③ The level of TSH was positive correlative with TC、TG、LDL-C、LPa、Hcy and UA(r=0.71、0.39、0.50、0.58、0.4、0.35,P <0.05).Conclusion The reason for lipid abnormal、Hcy and UA changing of patients with hypothyroidism may be relatived with the level of TSH and thy-roid function.Patients with hypothyroidism was susceptible to cardiovascular disease for the reason of increased LPa and Hcy.It is important for the hypothyroid patient to prevent cardiovascular disease under detecting the blood lipid, LPa and Hcy.
