目的:选择舒芬太尼和曲马多为研究药物,采用响应曲面分析法探讨阿片类一非阿片类镇痛药对术后伤口疼痛性刺激的药效学相互作用。方法选择 ASAI-II 级、年龄20-60岁手术患者70例,随机设定舒芬太尼浓度(0-1.8μg/ml,梯度间隔为0.2μg/ml),同时分别联合不同浓度曲马多(0-6.4 mg/ml,梯度间隔为0.8 mg/ml)。术终患者清醒后,以5ml/h 持续泵注不同浓度镇痛药,1 h 后观察患者静息下及体动下视觉模拟评分(visual analogue scale , VAS)。对两种药物镇痛效果间的相互作用进行响应曲面分析,并通过 NONMEM 软件求算响应曲面模型参数。结果舒芬太尼与曲马多相互作用的响应曲面分析表明,二者在抑制术后疼痛刺激具有明显的协同作用。结论响应曲面分析法成功揭示了两种药物镇痛效果间的作用规律。临床应用浓度范围内,舒芬太尼与曲马多在抑制术后疼痛刺激中表现出明显的协同作用,其中在抑制体动下 VAS 评分协同作用更为明显。

목적:선택서분태니화곡마다위연구약물,채용향응곡면분석법탐토아편류일비아편류진통약대술후상구동통성자격적약효학상호작용。방법선택 ASAI-II 급、년령20-60세수술환자70례,수궤설정서분태니농도(0-1.8μg/ml,제도간격위0.2μg/ml),동시분별연합불동농도곡마다(0-6.4 mg/ml,제도간격위0.8 mg/ml)。술종환자청성후,이5ml/h 지속빙주불동농도진통약,1 h 후관찰환자정식하급체동하시각모의평분(visual analogue scale , VAS)。대량충약물진통효과간적상호작용진행향응곡면분석,병통과 NONMEM 연건구산향응곡면모형삼수。결과서분태니여곡마다상호작용적향응곡면분석표명,이자재억제술후동통자격구유명현적협동작용。결론향응곡면분석법성공게시료량충약물진통효과간적작용규률。림상응용농도범위내,서분태니여곡마다재억제술후동통자격중표현출명현적협동작용,기중재억제체동하 VAS 평분협동작용경위명현。
Objective To describe the pharmacodynamic interactions between sufentanil and tramadol in postopera-tive analgesia with response surface analysis.Methods Seventy patients of ASA I-II,aged 20-60 years,were scheduled for the study.Each subjects was randomly received a series of different concentrations of sufentanil and tramadol with continuous intravenous analgesia 1 h(5 ml/h)after operation.The concentration of sufentanil was setted from 0ug/ml to 1.8 μg/ml (gradient intervale was 0.2 μg/ml),while that of tramadol was from 0mg/ml to 6.4mg/ml(gradient in-tervale was 0.8 mg/ml).At the end of intravenous infusion,all were received pain assessment with VAS(visual ana-logue scale).The pharmacodynamic interactions between sufentanil and tramadol were analyzed by response surface method.NONMEM was used to estimate the parameter values.Results The response surface analysis showed that sufentanil-tramadol interactions were strongly synergistic in Inhibition of postoperative pain stimulus.Conclusion Re-sponse surface method can analyze pharmacodynamic interactions between sufentanil and tramadol qualitatively and quantitatively.The interactions are strongly synergistic for blunting responses to postoperative analgesia.
