目的：评价中药保留灌肠在重症胰腺炎急性期治疗中的临床疗效与安全性。方法入选62例重症胰腺炎患者，随机分为试验组和对照组，各31例，对照组进行常规治疗，试验组在对照组基础上急性期辅以中药保留灌肠治疗，每天一剂，分两次，每次100 mL，10 d为1个疗程。比较2组临床症状（腹痛、腹胀缓解时间，排便恢复时间）、炎性因子（肿瘤坏死因子α、内毒素）、住院时间和治疗效果。结果试验组有效率为93.55％，明显高于对照组的74.19％（ P＜0.05）。试验组腹痛、腹胀缓解时间和排便恢复时间、住院时间、肿瘤坏死因子α、内毒素、并发症和死亡率明显低于对照组（ P＜0.05）。结论中药保留灌肠有助于改善急性重症胰腺炎临床症状，减轻炎性因子表达，提高治疗效果。
목적：평개중약보류관장재중증이선염급성기치료중적림상료효여안전성。방법입선62례중증이선염환자，수궤분위시험조화대조조，각31례，대조조진행상규치료，시험조재대조조기출상급성기보이중약보류관장치료，매천일제，분량차，매차100 mL，10 d위1개료정。비교2조림상증상（복통、복창완해시간，배편회복시간）、염성인자（종류배사인자α、내독소）、주원시간화치료효과。결과시험조유효솔위93.55％，명현고우대조조적74.19％（ P＜0.05）。시험조복통、복창완해시간화배편회복시간、주원시간、종류배사인자α、내독소、병발증화사망솔명현저우대조조（ P＜0.05）。결론중약보류관장유조우개선급성중증이선염림상증상，감경염성인자표체，제고치료효과。
Objective To investigated clinical effect and safety of tradi-tional Chinese medicine ( TCM) retention enema in the treatment of acute severe pancreas.Methods A total of 62 cases of severe acute pancreati-tis patients enrolled for the study were randomly divided into experiment group and control group , 31 patients in the control group received con-ventional treatment , patients in experiment group received the TCM retention enema on the basis of the conventional treatment , one dose a day, twice per 100 mL, 10 d course of treatment.Clinical symptoms ( abdominal bloating relief time , bowel recovery time ) , inflammatory factors(TNF-α, LPS), a therapeutic effect of the two groups were com-pared.Results The efficacy of experiment group was 93.55%, signifi-cantly higher than that of control group with efficacy 74.19%(P<0.05).Abdominal pain time, time to ease bloating, and bowel re-covery time , hospital stay , TNF-α, endotoxin ( LPS ) , complications and mortality of experiment group were significantly lower than those of the control group ( P <0.05 ) .Conclusion TCM retention enema is helpful to improve the clinical symptoms , reduce inflammatory factor expression and improve the effect of treatment .
