目的：分析双源CT多期增强扫描中肾脏透明细胞癌的影像学特征，以期提高对该病诊断的正确率。方法回顾性分析16例经病理证实的肾透明细胞癌在平扫和皮质期、实质期及肾盂期的双源CT增强表现。结果肾脏透明细胞癌以密度不均匀、假包膜、皮质期实质部分强化明显以及实质期肿瘤强化迅速减退、肾盂期肿瘤强化进一步减退为主要特点，直径<30mm的透明细胞癌动脉期呈全瘤性强化。结论肾脏透明细胞癌的双源CT多期增强扫描特征性表现有助于和其他肾脏肿瘤的鉴别诊断，扫描计划应包括平扫和皮质期、实质期及肾盂期增强扫描。
목적：분석쌍원CT다기증강소묘중신장투명세포암적영상학특정，이기제고대해병진단적정학솔。방법회고성분석16례경병리증실적신투명세포암재평소화피질기、실질기급신우기적쌍원CT증강표현。결과신장투명세포암이밀도불균균、가포막、피질기실질부분강화명현이급실질기종류강화신속감퇴、신우기종류강화진일보감퇴위주요특점，직경<30mm적투명세포암동맥기정전류성강화。결론신장투명세포암적쌍원CT다기증강소묘특정성표현유조우화기타신장종류적감별진단，소묘계화응포괄평소화피질기、실질기급신우기증강소묘。
Objective To analysis the imaging features of multiphasic enhancement of dual-source CT in the clear cel renal cel carcinoma (CCRCC),in order to elevate the diagnostic accuracy rate.Methods What the dual-source CT features of CCRCC during plain scan and dif erent enhancement phases such as cortical,parenchymal and pyelographic phases were proved with pathology in 16 cases were retrospectively analyzed.Results The density of CCRCC is inhomogeneity,pseudocapsules,substantial portion of the medul ary period enhanced obviously,and enhancement of CCRCC in vein phase was a lit le and lower than that of renal cortical,in parenchymal phase and pyelographic phases the tumor density dropped promptly as the main characteristic,30mm or less in diameter of CCRCC showed marked tumor enhancement in artery phase. Conclusion The imaging features of multiphasic enhancement of dual-source CT in CCRCC contribute to dif erential diagnosis with other renal tumor.The protocol must include plain scan and dif erent enhancement phases such as cortical,parenchymal and pyelographic phases.
