目的:应用波强度(WI)技术观察慢性心力衰竭(CHF)患者颈动脉血流动力学改变,同时通过分析WI技术各项参数与CHF患者常规超声测量指标及血清尿钠肽(BNP)之间的相关关系,探讨WI技术在评价CHF患者左室收缩功能中的作用。方法对32例CHF患者和30例年龄匹配的健康成人的右侧颈总动脉进行超声扫查,应用WI软件技术获取WI曲线,测量各参数,将两组参数进行比较。检测CHF患者BNP浓度值,并将BNP测值与WI参数进行相关性分析。结果与对照组比较,CHF组收缩早期峰值W1显著降低(<0.01),CHF组心电图R波到W1的时间间隔( R-1st)显著延长(<0.05),收缩早期峰W1至收缩晚期峰W2时间间隔(1st-2nd)显著减低(<0.05);CHF患者组W1与LVEF及BNP呈正相关。结论 WI技术可评价慢性心力衰竭患者左室收缩功能,为临床正确的诊断疾病及判断病情提供可靠的信息。

Objective To investigate the wave intensity(WI) technique to assess the systolic function of left ventricle in patients with Chronic heart failure(CHF). Methods Right common carotid artery of 32 patients with CHF and 30age-matched normal controls were examined with imaging technique of wave intensity. The fol owing parameters were measured: the first wave peak in early ejection (W1), the second wave peak in late ejection(W2),the interval between the R wave of the ECG and the first peak of W1(R-1st), the interval between the first peak and the second peak(1st-2nd),the ratio of R-1st and a cardiac cycle time (R-1st) and the ratio of 1st-2nd,as wel as one cardiac cycle time (1st-2ndHR).At the same time, the levels of BNP were measured in the group of CHF patients. Results In compared with the control group, the CHF group patients, W1 and the time(1st-2nd) were lower ( <0.05), while R-1st was higher ( <0.05) . The W1 was positive cor elated with LVEF and BNP in CHF group. Conclusion The WI technique could evaluate the left ventricular systolic function for patients with CHF.
