目的 评价三维各向同性脂肪抑脂快速自旋回波序列(3D-FS-TSE-SPACE)对踝关节韧带损伤的诊断价值. 方法 选取38例(男26例,女12例)临床怀疑踝关节韧带损伤患者,分别行踝关节横断位、冠状位及矢状位2D-TSE-T2WI和矢状位3D-PD-TSE-SPACE扫描,对3D-PD-TSE-SPACE图像进行横断位、冠状位的多平面重建.评估横断位、冠状位及矢状位2D-TSE-T2WI和矢状位3D-PD-TSE-SPACE图像对踝关节韧带的显示情况及损伤诊断. 结果 2D-TSE-T2WI及3D-PD-TSE-SPACE对踝关节距腓前韧带、跟腓韧带、距腓后韧带、胫舟韧带、胫跟韧带及胫距韧带损伤的诊断结果差异均无统计学意义(P> 0.05).3D-PD-TSE-SPACE的对踝关节韧带的解剖细节较2D-TSE-T2WI显示更加清楚. 结论 3D-PD-TSE-SPACE与MRI常规2D-TSE-T2WI的诊断效能一致,3D-PD-TSE-SPACE可以用于评价踝关节韧带损伤.

Objective To evaluate the diagnostic value of three-dimensional isotropic fat-suppressed turbo spin-echo sequence (3D-FS-TSE-SPACE) for ankle ligament injuries.Methods MRI conventional sequences (including 2D-TSE-T2WI at coronal,sagittal,and axial positions) and 3D-FS-TSE-SPACE were used to examine the ankle joints of 38 patients (26 males and 12 females) who were suspected of ligament injuries.Multiplanar reconstruction was performed of the 3D-FS-TSE-SPACE at coronal and axial positions after examination.The images of 2D-TSE-T2WI at coronal,sagittal,and axial positions and sagittal 3D-PD-TSE-SPACE were compared in the display and diagnosis of ankle ligament injuries.Results There were no significant differences in the diagnosis of the anterior talofibular ligament,calcaneofibular ligament,posterior talofibular ligament,tibionavicular ligament,tibiocalcaneal ligament and tibiotalar ligament between 2D-TSE-T2WI and 3D-FS-TSE-SPACE (P > 0.05).The latter displayed the anatomic details of the ankle ligaments more clearly than the former.Conclusion Since 3D-PD-TSE-SPACE is similar to 2D-TSE-T2WI in the diagnosis of ankle ligament injuries,it can be used to evaluate ankle ligament injuries.
