Bilevel positive airway pressure non-invasive ventilator%Chronic obstructive pulmonary disease%Pulmona-ry failure
目的:观察双水平气道正压通气( BiPAP)呼吸机辅治对慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的效果。方法将60例COPD合并Ⅱ型呼衰患者随机分成治疗组和对照组,对照组给予常规抗感染,平喘化痰、持续低流量吸氧、营养支持等综合治疗;治疗组除了常规治疗外,加予无创机械通气治疗。观察和对比2组患者治疗后0、2、24和72h的动脉血气分析( SaO2、pH、PaO2、PaCO2)及生命体征的变化,比较气管插管率、住院病死率、平均住院日指标。结果治疗组30例创通气治疗后pH值上升,SaO2及PaO2上升,PaCO2下降,患者住院病程缩短,气管插管率明显下降,与对照组比较差异有统计学意义(P<0.05)。2组住院病死率无统计学意义(P>0.05)。结论 COPD合并Ⅱ型呼吸衰竭早期应用双水平气道正压通气辅治可明显改善患者的通气,降低气管插管率和缩短住院时间。

목적:관찰쌍수평기도정압통기( BiPAP)호흡궤보치대만성조새성폐질병합병Ⅱ형호흡쇠갈적효과。방법장60례COPD합병Ⅱ형호쇠환자수궤분성치료조화대조조,대조조급여상규항감염,평천화담、지속저류량흡양、영양지지등종합치료;치료조제료상규치료외,가여무창궤계통기치료。관찰화대비2조환자치료후0、2、24화72h적동맥혈기분석( SaO2、pH、PaO2、PaCO2)급생명체정적변화,비교기관삽관솔、주원병사솔、평균주원일지표。결과치료조30례창통기치료후pH치상승,SaO2급PaO2상승,PaCO2하강,환자주원병정축단,기관삽관솔명현하강,여대조조비교차이유통계학의의(P<0.05)。2조주원병사솔무통계학의의(P>0.05)。결론 COPD합병Ⅱ형호흡쇠갈조기응용쌍수평기도정압통기보치가명현개선환자적통기,강저기관삽관솔화축단주원시간。
Objective To study the clinical therapeutic effect of using bilevel positive airway pressure non-invasive ventilator on chronic obstructive pulmonary disease and pulmonary failure. Methods The total of 60 patients with chronic ob-structive pulmonary disease and pulmonary failure were divided into experimental group( 30 case ) and control group( 30 case). the routine therapy was given in two groups,but non-invasive ventilator and routine therapy both were given in experi-mental group. The blood gas analysis was evaluated after0h,2h,24h and 72h,the changes of arterial pH,PaCO2 ,PaO2 ,SaO2 and clinical symptoms were compared between the two groups,and also intubation rate,death rate,hospitalization time. Re-sults There was no signnificant difference of the two group patients′condition at the time of admission(P<0. 05). There was a progressive improvement in HR/RR,pH,PaCO2 ,PaO2 ,SaO2 were observe in experimental group. Comparing two groups,the average hospitalization time and the intubation rate were lower in experimental group(P<0. 05),but no significance compared with the death rate in two groups(P>0. 05). Conclusion Bilevel positive airway pressure non-invasive ventilator can improve the therapeutic effect of patients with chronic obstructive pulmonary disease and pulmonary failure. The method was an alterna-tive choice for stable patients needing prolonged mechanical ventilation.
