目的:探讨环氧化酶-2基因( COX-2 mRNA)在骨关节炎性病变中的变化及其作用。方法以风湿免疫科2010至2014年收治的51例膝骨性关节炎患者(病例组)及20例非膝骨性关节炎患者(对照组)为研究对象,根据膝骨性关节炎的病变进程分为早期组(17例)、中期组(19例)和晚期组(15例),分别采用实时荧光定量法(RT-PCR)检测各组患者的成纤维样滑膜细胞中COX-2mRNA表达水平、采用Western-blot法检测各组COX蛋白表达水平。结果病例组患者成纤维细滑膜样细胞中COX-2mRNA及蛋白表达灰度值分别为0櫃.76±0.11、0.85±0.09均显著高于对照组( P <0.05)。早期组、中期组、晚期组患者和对照组成纤维细滑膜样细胞中COX-2mRNA及蛋白表达灰度值差异有统计学意义( P <0.05);早期组COX-2mRNA及蛋白表达灰度值分别为0.92±0.09、0.98±0.09,均显著高于中期组、晚期组患者和对照组( P <0.05);中期组、晚期组患者COX-2mRNA及蛋白表达灰度值差异无统计学意义( P >0.05);中期组、晚期组患者COX-2mRNA及蛋白表达灰度值显著高于对照组( P <0.05)。结论膝骨性关节炎患者成纤维细滑膜样细胞中COX-2mRNA及蛋白表达水平显著高于非股性关节炎患者,同时表达水平早期最高,说明环氧化酶-2基因可能主要作用于膝骨性关节炎患者炎症病变的早期。

Objective To observe the changes of cyclooxygenase-2 gene ( COX-2 mRNA ) in the process of inflammatory lesion of bone and joint, and to explore its effects on the lesions.Methods Fifty -one patients with knee osteoarthritis ( case group) and 20 patients with non knee osteoarthritis ( control group) who were admitted into Department of Rheumatism Immunization Diseases of our hospital from 2010 to 2014 were enrolled in the study.According to the process of inflammatory lesion of bone and joint, these patients were redivided into early period group ( n =17) , middle period group (n=19) and late period group ( n =15).The expression levels of COX-2mRNA in fibroblast like synovial cells were detected by Real-time fluorescence quantitative PCR( RT-PCR) ,and the expression levels of COX protein were detected by Western Blot.Results The expression gray values of COX-2 mRNA and protein in fibroblast like synovial cells in case group were 0.76 ±0.11,0.85 ±0.09,respectively, both of them were significantly higher than those in control group ( P <0.05). There were significant differences in the expression gray values of COX-2mRNA and protein among early period group,middle period group,late period group ( P <0.05).The expression gray values of COX-2mRNA and protein in early period group were 0.92 ±0.09,0.98 ±0.09,respectively, both of them were significantly higher than those in middle period group,late period group and control group ( P<0.05),however, there were no significant differences in the expression gray values of COX-2mRNA and protein between middle period group and late period group ( P >0.05 ) .The expression gray values of COX-2mRNA and protein in middle period group and late period group were significantly higher than those of control group (P<0.05).Conclusio n The expression gray values of COX-2mRNA and protein in fibroblast like synovial cells in patients with knee osteoarthritis are significantly higher than those of patients with non knee osteoarthritis,moreover, the expression levels are the highest at early period,which suggests that cyclooxygenase-2 gene may play an important role in early period of inflammatory lesions of bone and joint in patients with knee osteoarthritis.
