目的：探讨腹膜后 Castleman 病(Castleman′s disease,CD)的临床特点及治疗方法。方法回顾分析1例腹膜后肾上腺区巨大CD患者的临床资料,并结合文献进行讨论。患者以反复口腔溃疡症状就诊,术前有CT等影像学表现,未能明确诊断,肿块位于左肾旁及左肾上腺区,拟采用经腹手术探查。结果患者手术顺利,术后病理证实为腹膜后巨大 CD,血管滤泡型增生,术后患者周身皮损及口腔溃疡症状消失,随访1年无肿瘤复发。结论腹膜后 CD临床罕见,临床医生多缺乏对此病的系统认识,术前诊断困难,确诊主要依靠病理和免疫组化检查,手术切除肿瘤是首选治疗方式,有望达到治愈目的。
목적：탐토복막후 Castleman 병(Castleman′s disease,CD)적림상특점급치료방법。방법회고분석1례복막후신상선구거대CD환자적림상자료,병결합문헌진행토론。환자이반복구강궤양증상취진,술전유CT등영상학표현,미능명학진단,종괴위우좌신방급좌신상선구,의채용경복수술탐사。결과환자수술순리,술후병리증실위복막후거대 CD,혈관려포형증생,술후환자주신피손급구강궤양증상소실,수방1년무종류복발。결론복막후 CD림상한견,림상의생다결핍대차병적계통인식,술전진단곤난,학진주요의고병리화면역조화검사,수술절제종류시수선치료방식,유망체도치유목적。
Objective To discuss the clinical characteristics and treatment methods of retro-peritoneal Castleman's disease (CD). Methods We retrospectively analyzed 1 case of huge retro-peritoneal Castleman tumor located in adrenal area in our hospital in 2014.This patient had a repeat-ed oral ulceration and CT scan shows that the mass locates in left adrenal area.Consequently the pa-tient received surgery treatment. Results Patients were confirmed by pathology huge retroperito-neal lymph tissue hyperplasia,vascular follicular hyperplasia.After surgery this patient was followed up for 1 year and there was no tumor and ulceration recurrence. Conclusions Retroperitoneal CD is a rare tumor,which is difficult in preoperative diagnosis.The final diagnosis mainly depends on the pathology and immunohistochemical examination.Surgery is the most preferred treatment to the dis-ease.
