目的：比较连续硬膜外麻醉与腰－硬联合麻醉在剖宫产手术中的临床价值及效果。方法选择符合条件的剖宫产手术产妇96例，采用随机数表法分为连续组与联合组，每组各48例，其中连续组采取连续硬膜外麻醉下行剖宫产，联合组采取腰－硬联合麻醉下行剖宫产，比较两组患者的麻醉效果、不良反应发生率及新生儿评分等指标。结果联合组产妇麻醉起效时间、诱导时间及手术用时明显少于连续组（ P＜0．05）；联合组与连续组新生儿评分比较，差异无统计学意义（P＞0．05）；联合组麻醉满意度高于连续组（P＜0．05）；连续组不良反应发生率（23．0％）明显高于联合组（2．1％），差异有统计学意义（ P＜0．05）。结论剖宫产手术采用腰－硬联合麻醉可提高麻醉满意度、麻醉起效快、镇静效果强，不良反应发生率较低，值得临床推广使用。
목적：비교련속경막외마취여요－경연합마취재부궁산수술중적림상개치급효과。방법선택부합조건적부궁산수술산부96례，채용수궤수표법분위련속조여연합조，매조각48례，기중련속조채취련속경막외마취하행부궁산，연합조채취요－경연합마취하행부궁산，비교량조환자적마취효과、불량반응발생솔급신생인평분등지표。결과연합조산부마취기효시간、유도시간급수술용시명현소우련속조（ P＜0．05）；연합조여련속조신생인평분비교，차이무통계학의의（P＞0．05）；연합조마취만의도고우련속조（P＜0．05）；련속조불량반응발생솔（23．0％）명현고우연합조（2．1％），차이유통계학의의（ P＜0．05）。결론부궁산수술채용요－경연합마취가제고마취만의도、마취기효쾌、진정효과강，불량반응발생솔교저，치득림상추엄사용。
Objective To study the comparison of appIication effects between continuous epi-duraI anesthesia and waist hard joint anesthesia in cesarean section surgery.Methods 96 cases under the cesarean section surgery from January to September,2013 in our hospitaI were ran-domIy divided into continuous and joint group, 48 cases in each group.The continuous group was treated by the continuous epiduraI anesthesia in the cesarean section surgery,but the joint group took the waist hard joint anesthesia.The anesthesia effects, incidence of adverse reac-tions and newborn grading of the two groups were compared.Results There was great differ-ence among anesthesia working time,induction time and surgery time.The joint group had an advantage in above time (P<0.05).The Apgar method was used to assess neonataI outcomes quaIity,which showed the scores of the two groups had no significant difference (P>0.05).But the anesthesia satisfaction and fetaI chiIdbirth time of the joint group were obviousIy better than those of the continuous group ( P<0.05 ) .Conclusion The waist hard joint anesthesia used in ce-sarean section surgery can improve anesthesia satisfaction and anesthesia effects and obtain stronger sedative effects and Iower incidence of adverse reactions.It has great cIinicaI appIica-tion and popuIarization vaIues.
