我国已进入老龄化社会，老年痴呆症是常见的老年神经精神疾病，严重影响患者和照顾者的生活质量，给家庭和社会带来沉重的负担。因老年痴呆症患者患病后病症表现特殊，需要长期照顾，且投入的人力物力比普通老年人更大。社会工作中的个案管理模式可以为老年痴呆症患者制定全方位的服务计划，很好地满足这种类型老年人的多重需求。

China has entered into an aging society.Senile dementia has become one of the common diseases,which badly affect the health and life quality of the aged people as well as their caregivers. This kind of disease has also imposed a heavy burden on the patients’families and society.Because of the special symptoms of this disease,the patients need long-term care,thus tending them will require more manpower and material resources.The model of case management in social work can meet multi-ple needs of the dementia patients by making a comprehensive service plan.This paper takes Guangxi Social Welfare Institute as an example,discussing how to apply case management to deal with the aged dementia who are taken care in nursing homes.
