目的 评价人乳头瘤病毒(HPV)疫苗按2剂次免疫程序(2 d)和3剂次免疫程序(3 d)接种后免疫学效果的差异.方法 电子检索美国国家医学图书馆数据库(NCBI)、考克兰协作网图书馆(Cochrane)等数据库,将有关比较按3d和2d接种HPV疫苗免疫学效果的研究纳入分析.通过计算抗体几何平均浓度(GMC)比值来比较按3d和2d接种HPV疫苗后产生针对HPV-16和HPV-18抗体的差异.采用Meta分析合并二价HPV按3d和2d接种后产生的抗体GMC比值.结果 共纳入5篇文献.9~14岁女性按3d与15~26岁女性按2d接种HPV疫苗后针对HPV-16和HPV-18的抗体GMC比值的95%CI上限均未超过非劣效性研究界值2.0;同年龄组女性按3d和2d接种HPV后有部分亚组的抗体GMC比值的95%CI超过2.0;9~14岁女性按2d和3d接种二价HPV疫苗24月后针对HPV-16抗体的GMC比值的95%CI超过2.0.结论 9~14岁女性按2d接种HPV疫苗与15~26岁女性按3d接种HPV产生的免疫学效果相当,在不同随访时间点上未发现采用2d与3d接种HPV疫苗产生的免疫学效果存在差异.

Objective To evaluate the difference on immunogenicity of 3 doses schedule(3 d) and 2 doses schedule(2 d) of human papillomavirus (HPV) vaccine.Methods By searching database such as "National Center for Biotechnology Information" and "Cochrane Library",the studies of comparing immunogenicity of 3 d and 2 d of HPV vaccination were included.Geometric mean concentrations (GMC) ratios(GMC after 3 d/GMC after 2 d) of HPV-16 and HPV-18 between 3 d and 2 d were calculated.Meta-analysis was adopted to combine pooled GMC ratios (bivalent vaccine) of 3 d and 2 d.Results A total of 5 studies were included.The 95% CI GMC ratio against HPV-16 and HPV-18 between the 2 d in women aged 15-26 years and the 3 d in women aged 9-14 years did not exceed the non-inferiority (NI) margin of 2.0.The 95% CI of GMC ratios against HPV-16 and HPV-18 between the 3 d and the 2 d in some equivalent age group exceeded 2.0.The 95% CI of pooled GMC ratios for the bivalent HPV within women aged 9-14 years between the 3d and the 2 d exceeded 2.0 at 24 months after the last dose.Conclusions The 2 d of women aged 9-14 years has non-inferior immunogenicity compared with the 3 d of women aged 15-26 years.However,there is no difference at various timing of vaccinating HPV by 2d and 3d.
