目的：探讨腹腔镜胆囊切除术中副右肝管损伤的预防及术中处理方法。方法：回顾性分析我院近10年收治的1842例腹腔镜胆囊切除术中出现副右肝管损伤4例患者的临床资料，总结4例患者副右肝管的变异类型、处理方法及预后。结果：4例患者均于术中造影证实为副右肝管，采用术中结扎的方式进行处理，4例患者均术后恢复良好，随访均无远期并发症发生。结论：为防止在腹腔镜胆囊切除术中损伤副右肝管，应熟悉胆管变异的各种类型、正确解剖胆囊三角、合理应用术中胆道造影，对于明确为副右肝管损伤的患者，结扎处理安全有效。
목적：탐토복강경담낭절제술중부우간관손상적예방급술중처리방법。방법：회고성분석아원근10년수치적1842례복강경담낭절제술중출현부우간관손상4례환자적림상자료，총결4례환자부우간관적변이류형、처리방법급예후。결과：4례환자균우술중조영증실위부우간관，채용술중결찰적방식진행처리，4례환자균술후회복량호，수방균무원기병발증발생。결론：위방지재복강경담낭절제술중손상부우간관，응숙실담관변이적각충류형、정학해부담낭삼각、합리응용술중담도조영，대우명학위부우간관손상적환자，결찰처리안전유효。
Objective:To summarize our experience in the prevention and treatment of right accessory he‐patic duct injury during laparoseopic cholecystectomy .Methods:The clinical data of 4 with right accessory hepatic duct during laparoscopic cholecystectomy were reviewed retrospectively .Results :According to anatomy identified by preoperative work up and selective cholangiography during the operation ,4 Cases had the right accessory hepatic duct ,all of them were confirmed intraoperatively .All patients were underwent right accessory hepatic duct ligation , no postoperative complications were found .Conclusion:To prevent injury of right accessory hepatic duct .High vigi‐lance and familiarity with the anatomic variants of the biliary tree and intraoperative cholangiography in selective ca‐ses are fundmental .Ligation of the right accessory hepatic duct is safe and effective method for the patients who intr‐aoperative diagnosis .
