目的：分析急性胰腺炎（A P ）患者血液淋巴细胞和中性粒细胞的动态变化及意义。方法：检测668例A P患者血液淋巴细胞和中性粒细胞计数。结果：A P患者淋巴细胞和中性粒细胞发生了动态规律性变化。AP初期淋巴细胞降低，中性粒细胞升高，从对照组到轻度 AP、中度 AP和重度 AP（SAP）逐渐变化（P＜0．05）。AP病情、型别越重，淋巴细胞降低幅度越大，中性粒细胞升高幅度越大。结论：AP淋巴细胞和中性粒细胞发生了动态规律性变化，其变化程度与AP病情的严重程度有统计学意义，淋巴细胞计数变化提示有免疫功能的变化。动态检测淋巴细胞和中性粒细胞有助于AP的临床分型、预后判断和病程进展分期，为治疗提供依据。

Objective:To investigate dynamical change and significance of lymphocyte and polymorphonu‐clear neutrophilsi(PMN) in acute pancreatitis (AP) .Methods:To detect blood lymphocyte and polymorphonuclear neutrophils count of 668 cases of AP patients in our hospital from Jan 2009 to Jun 2014 .Results :Lymphocyte and PMN changed dynamically and regularly .In initial stage of AP ,lymphocyte decreased and PMN increased gradually from CC group to MAP ,MSAP ,to SAP ,there was statistical significance (P<0 .05) .The more serious the condi‐tion and grade of AP were ,the more lymphocyte decreased and PMN increased .Conclusion:Lymphocyte and PMN change dynamically and regularly ,and there is statistical significance (P<0 .05) between the change severity and the condition severity of AP .Change of lymphocyte count implies immune function change .Dynamical detection of lym‐phocyte and PM N is helpful for clinical classification ,prognosis evaluation and course staging ,consequently provides the basis of treatment for AP .
