Nursing grading standards%Nursing level%Accuracy
目的：通过调查住院患者实际医嘱护理分级和依据新护理分级标准确定的护理分级的差异性，了解医嘱护理级别下达的准确性，对存在的缺陷进行分析。方法：整群抽取全院某一天住院患者中Barthel指数评定总分≤40分的患者115例，并对他们进行重新评分，比较护士评分的准确性及与实际医嘱护理级别的相符性。结果：医护对住院患者护理级别的制定差异有统计学意义(P<0.01)，一级护理医嘱内外科之间差异有统计学意义(P<0.01)。结论：建议加强对医生进行新护理分级标准的培训，加强护理分级医嘱管理，医护共同决策，以保证护理分级制度执行的正确性、科学性及可操作性，保障护理质量的落实。

목적：통과조사주원환자실제의촉호리분급화의거신호리분급표준학정적호리분급적차이성，료해의촉호리급별하체적준학성，대존재적결함진행분석。방법：정군추취전원모일천주원환자중Barthel지수평정총분≤40분적환자115례，병대타문진행중신평분，비교호사평분적준학성급여실제의촉호리급별적상부성。결과：의호대주원환자호리급별적제정차이유통계학의의(P<0.01)，일급호리의촉내외과지간차이유통계학의의(P<0.01)。결론：건의가강대의생진행신호리분급표준적배훈，가강호리분급의촉관리，의호공동결책，이보증호리분급제도집행적정학성、과학성급가조작성，보장호리질량적락실。
Objective:To explore the accuracy of the nursing level and analyze the defects according to the investigation of the differences between the actual nursing classification in hospitalized patients and the nursing level by new nursing grading standards.Method:115 hospitalized patients with Barthel index scores less then 40 on a certain day were extracted in the whole.The Barthel index scores were graded again.The consistency between the scores assessed by the nurse and actual nursing level was analyzed.Result:The differences in the nursing level existed between doctors and nurses(P<0.01).The difference of grade one nursing in internal medicine and in surgery was also statistically significant(P<0.01).Conclusion:The training of new nursing grading standards is strongly suggested in doctors.The management of nursing grading the doctor’s advice and co-determination between doctors and nurses should be strengthened,which may ensure the correctness,scientificity and operability of the execution of nursing classification and safeguard the quality of nursing.
