Acinetobacter baumannii%Infection%Drug-resistance%ICU
目的:探讨ICU病房鲍曼不动杆菌感染情况及耐药性。方法回顾性分析2012年1月~2013年12月在西安交通大学医学院第二附属医院ICU治疗的86例鲍曼不动杆菌培养阳性的患者资料,对其预后、标本类型及耐药情况进行分析。结果 ICU鲍曼不动杆菌的感染率为3.8%,死亡率为23.3%(20例),存活率为76.7%(66例),存活组的年龄小于死亡组,入住ICU时间长于死亡组,差异有统计学意义(P<0.05)。所有患者标本以呼吸道痰液标本来源最多,占68.6%(59/86),86株鲍曼不动杆菌对氨苄西林的耐药率最高达93.0%,对青霉素、头孢类耐药率为60%及以上。结论 ICU鲍曼不动杆菌耐药情况严重,临床应该监测鲍曼不动杆菌耐药情况,合理使用抗菌药物,减少耐药鲍曼不动杆菌株的出现。

목적:탐토ICU병방포만불동간균감염정황급내약성。방법회고성분석2012년1월~2013년12월재서안교통대학의학원제이부속의원ICU치료적86례포만불동간균배양양성적환자자료,대기예후、표본류형급내약정황진행분석。결과 ICU포만불동간균적감염솔위3.8%,사망솔위23.3%(20례),존활솔위76.7%(66례),존활조적년령소우사망조,입주ICU시간장우사망조,차이유통계학의의(P<0.05)。소유환자표본이호흡도담액표본래원최다,점68.6%(59/86),86주포만불동간균대안변서림적내약솔최고체93.0%,대청매소、두포류내약솔위60%급이상。결론 ICU포만불동간균내약정황엄중,림상응해감측포만불동간균내약정황,합리사용항균약물,감소내약포만불동간균주적출현。
Objective To investigate the infection and drug-resistance of Acinetobacter baumannii of patients in inten-sive care unit (ICU). Methods The clinical data of 86 patients whose Acinetobacter baumannii cultivation was positive treated in ICU of the Second Affiliated Hospital of Xi'an Jiaotong University form January 2012 to December 2013 was analyzed retrospectively. The prognosis, specimen type and drug resistance were analyzed. Results In ICU, the infection rate of Acinetobacter baumannii was 3.8%, the mortality rate was 23.3% (20 cases), while the survival rate was 76.7%(66 cases). Age of the survival group was smaller than that of the death group, the ICU stay was longer than that of the death group, the differences were statistically significant (P<0.05). Among all the specimens, respiratory sputum spec-imens were the most, which accounted for 68.6% (59/86). The highest drug resistant rate of 86 strains of Acinetobacter baumannii to Ampicillin was 93.0%, the drug resistant rates for Penicillin and Cephalosporin were more than 60%. Conclusion In ICU, the drug-resistance of Acinetobacter baumannii is heavy, in clinic, the drug-resistance of Acine-tobacter baumannii should be monitored, so as to use antibacterial agents reasonably and reduce the occurrence of drug-resistant Acinetobacter baumannii.
