目的探讨内斜视A征的临床特征、手术方式及治疗效果.方法回顾性分析2010年1月至2014年8月在兰州市第一人民医院眼科采用手术治疗的内斜视A征38例,依据不同的临床表现,对伴有上斜肌功能亢进者行上斜肌减弱术,反之则行内直肌后徙并上移术,观察手术前、后患者眼位、斜视度和临床特征的变化.结果 38例内斜视A征患者中,上斜肌功能亢进34例64只眼,行上斜肌减弱术后60只眼上斜肌亢进消失;术后眼位恢复至正位32例,A征消失;术后19例患者恢复了双眼不同程度的单视功能.结论内斜视A征的主要发生原因和临床特征为上斜肌功能亢进;矫正内斜视A征的有效手术治疗方式是上斜肌减弱术和内斜视矫正术;手术治疗内斜视A征在矫正眼位的同时,还可以使部分患者的双眼单视功能得到重建.

Objective To investigate the clinical features and the surgical method of A-pattern esotropia and to evaluate the efficacy of surgical treatment.Methods A retrospective study of 38 patients with A-pattern esotropia was reviewed.According the different clinical manifestations,the patients with overaction of the superior oblique (SO) were underwent the SO tenectomy surgery,the patients with normal function of the SO were underwent the medial rectus migration surgery and move up.Results In the group of 38 patients,there were 34 patients who were diagnozed overaction of the SO.The overaction of the SO (60 eyes) were disappeared after the SO tenectomy.84.21％ (32/38) of A-patteren esotropia in our study were successfully corrected.Niteen cases obtained binocular function after surgery.Conclusions The primary factor in the A-pattern esotropia is the SO overaction.The effective way correct A-pattern esotropia is weakening the overaction of the SO combined with the correction of esotropia.The surgery can correct the eye position and recovery the single vision function of some patients by exercise.
