目的观察玻璃体切割联合C3F8气体填充术治疗孔源性视网膜脱离的疗效,评价该术式的安全性及有效性.方法对2012年9月至2013年9月在中国医科大学附属第一医院眼科随访观察孔源性视网膜脱离病人40例40只眼,均行标准三切口经睫状体平坦部玻璃体切割联合惰性气体填充,观察患者最佳矫正视力恢复情况;眼部B超、眼底照片示视网膜复位情况;采集低视力生存质量量表.结果术后定期随访,通过眼底检查,术眼脱离的视网膜全部复位,至随访结束,视网膜复位良好,术前BCVA的平均值为(1.51±0.64) LogMAR,术后1个月(0.87±0.26) LogMAR,术后3个月和6个月为(0.62±0.25) LogMAR.术后1、3、6个月BCVA的LogMAR与术前比较,差别均有统计学意义(P＜0.05).术前量表总分为88.36±36.38,术后量表总分为110.47±45.28,手术前后总分差值为22.11±9.45,手术前后量表总分及平均分比较,差别有统计学意义(P＜0.05).结论玻璃体切割联合惰性气体C3F8玻璃体腔内填充术治疗孔源性视网膜脱离疗效确切,同时减轻了患者长期面朝下体位的痛苦,并能提高患者生存质量.
목적 관찰파리체절할연합C3F8기체전충술치료공원성시망막탈리적료효,평개해술식적안전성급유효성.방법 대2012년9월지2013년9월재중국의과대학부속제일의원안과수방관찰공원성시망막탈리병인40례40지안,균행표준삼절구경첩상체평탄부파리체절할연합타성기체전충,관찰환자최가교정시력회복정황;안부B초、안저조편시시망막복위정황;채집저시력생존질량량표.결과 술후정기수방,통과안저검사,술안탈리적시망막전부복위,지수방결속,시망막복위량호,술전BCVA적평균치위(1.51±0.64) LogMAR,술후1개월(0.87±0.26) LogMAR,술후3개월화6개월위(0.62±0.25) LogMAR.술후1、3、6개월BCVA적LogMAR여술전비교,차별균유통계학의의(P＜0.05).술전량표총분위88.36±36.38,술후량표총분위110.47±45.28,수술전후총분차치위22.11±9.45,수술전후량표총분급평균분비교,차별유통계학의의(P＜0.05).결론 파리체절할연합타성기체C3F8파리체강내전충술치료공원성시망막탈리료효학절,동시감경료환자장기면조하체위적통고,병능제고환자생존질량.
Objective To observe the effectiveness and safety of the vitrectomy combined with C3F8 gas tamponade on treatment of rhegmatogenous retinal detachment.Methods Forty cases (40 eyes) of rhegmatogenous retinal detachment were followed up and observed after underwent vitrectomy combined with inert gas tamponade combined with laser photocoagulation.The visual acuity recovery,eye fundus photographs were observed,and B ultrasound checked retinal reattachment.Low vision quality of life questionnaire were collected.Results After the operation,through fundus examination the detached retina were reattached.Average value of the best corrected visual acuity(Log-MAR):preoperative was 1.51±0.64;1 month after the operation was 0.87±0.26;3 months and 6 months after the operation was 0.62±0.25.Compared with the preoperative,there was difference of postoperative (1 month,3 mouths,and 6 mouths) at visual acuity (P ＜0.05).The total score of preoperative gauge was 88.36±36.38,after surgical intervention,the total score was 110.47±45.28.Compared with preoperative,there was difference of postoperative in total score and average score.Conclusions Vitrectomy combined with intravitreal C3F8 inert gas tamponade for the treatment of rhegmatogenous retinal detachment is obvious curative effect,at the same time reduce the patients' pain of face down position in long-term,can improve the quality of patients' life.
