①目的探讨甲状腺癌患者心理健康水平和人格特征。②方法采用症状自评量表（SCL－90）对60例甲状腺癌患者术前和术后心理健康状况进行测量。术前采用艾森克个性问卷（EPQ ）对患者人格状况进行测量。对测试结果进行 t检验和相关分析。③结果术前SCL －90总分、躯体化、抑郁、焦虑、敌对、恐怖、偏执、精神病性因子得分高于常模（t ＝－159．6、－26．0、443．2、397．2、－32．9、375．6、－20．1、22．1，P ＜00．5），术后SCL －90总分、躯体化、强迫、人际敏感、抑郁、焦虑、敌对、恐怖、偏执、精神病性因子得分高于手术前（t ＝412．5、159．4、79．8、101．1、684．0、716．2、132．3、924．5、88．2、2．26，P ＜00．5）。甲状腺癌SCL －90总分、躯体化、强迫、人际敏感、抑郁、焦虑、敌对、恐怖、偏执因子得分手术后高于手术前（ t ＝－353．2、－301．9、－10．38、－181．6、－142．2、－196．3、－190．8、－166．3、－216．5， P ＜00．1）。甲状腺癌男患者EPQ的精神质、神经质得分高于常模（ t ＝359．4、454．0，P ＜00．01），内外向得分低于常模（ t ＝－71．7，P ＜00．01），甲状腺癌女患者EPQ的精神质、神经质得分高于常模（ t ＝382．5、418．7，P ＜00．01），内外向得分低于常模（ t ＝－102．7，P ＜00．01）。甲状腺癌患者内外向与SCL －90抑郁、焦虑和恐怖因子呈负相关（ r ＝－03．8、－039、－03．7，P ＜00．1），神经质与SCL －90总分、抑郁、焦虑和恐怖因子呈正相关（r ＝03．8、06．6、06．7、06．4，P ＜00．1）。④结论甲状腺癌患者手术前后心理健康问题明显，其与人格有关，人格缺陷可能促发甲状腺癌。

Objective To investigate mental health status and personality of patients with thyroid cancer .The influence of personality on mental health and personality defect may be one of the factors of thyroid carcinoma .Methods We examined both the preoperative and postoperative mental state of 60 patients according to the symptom check list -90(SCL -90) ,and used the Eysenck Personality Ques‐tionnaire (EPQ ) in the preoperative measurement ,t test and correlation analysis to the result of the test .Results ①The preoperative SCL -90 total scores ,somatization ,depression ,anxiety ,hostility ,ter‐ror ,paranoia ,psychosis factor scores are higher in case group than control group( t = -15 9.6 ,-2 6.0 , 44 3.2 ,39 7.2 ,-3 2.9 ,37 5.6 ,-2 0.1 ,2 2.1 ,P <0 0.5) ,The postoperative SCL -90 total scores ,somatiza‐tion ,compulsion ,interpersonal sensitivity ,depression ,anxiety ,hostile ,terror ,paranoid ,psychosis factor scores are higher n case group than control group( t =41 2.5 ,15 9.4 ,7 9.8 ,10 1.1 ,68 4.0 ,71 6.2 ,13 2.3 ,92 . 45 ,8 8.2 ,2 2.6 ,P <0 0.5);②The SCL -90 total scores ,somatization ,compulsion ,interpersonal sensitiv‐ity ,depression ,anxiety ,hostility ,terror ,paranoia factor scorers in postoperative group are higher than preoperative group( t = -35 3.2 ,-30 1.9 ,-10 3.8 ,-18 1.6 ,-14 .22 ,-19 6.3 ,-19 .08 ,-16 .63 ,-21 . 65 ,P < 0 .01);③ The EPQ psychoticism and neurosis scores in male thyroid carcinoma patients are higher than control group( t =35 9.4 ,45 4.0 ,P <0 0.01) ,while that introversion and extroversion scores lower than control group ( t = -7 1.7 ,P <0 0.01);The EPQ psychoticism and neurosis scores in fe‐male thyroid carcinoma patients are higher than control group ( t =38 2.5 ,41 8.7 ,P <0 0.01) ,w hile that introversion and extroversion scores lower than control group ( t = -10 2.7 ,P <0 0.01);④Thyroid carcinoma patients'introversion and extroversion scores are negatively correlated with SCL -90 depres‐sion ,anxiety and terror factor scores (r= -0 3.8 ,-039 ,-0 3.7 ,P<0 0.1) ,Thyroid carcinoma patients'neurosis are positively correlated with SCL -90 total scores ,depression ,anxiety and terror factor scores( r =0 3.8 ,0 6.6 ,0 6.7 ,0 6.4 ,P <0 0.1) .Conclusion There are obvious psychological and personali‐ty problems with thyroid cancer patients before and after having operations .Psychological problems are related to personality .Personality defects may contribute to thyroid cancer .
