目的 探讨不同形态早期年龄相关性白内障对客观视觉质量的影响.方法 临床病例对照研究.选取2013年3月到2013年8月就诊于中国医科大学眼科医院的最佳矫正视力(BCVA) Log-MAR≤0.3的年龄相关性白内障患者113例(121只眼),分为3组,其中皮质性白内障组41例(44只眼)、核性白内障组36例(39只眼)、后囊膜下白内障组36例(38只眼).使用综合验光仪测量患者的BCVA,OQAS视觉质量分析仪测量患者的散射光(OSI),调制传递函数(MTF cut off)和斯特列尔比(SR).结果 三组的BCVA (LogMAR)分别为:皮质性白内障组(0.23±0.10) logMAR,核性白内障组(0.23±0.08) logMAR,后囊膜下白内障组(0.25±0.10) logMAR,三组BCVA的差异无统计学意义(F =0.68,P=0.51).三组的年龄、眼内散射光的自然对数LN (OSI)、MTF (cut off),斯特列尔比(SR):皮质性白内障组分别为64.57±6.25、1.35±0.63、11.85±4.24、0.09±0.02,核性白内障组:分别为69.90±5.01、1.01 ±0.43、14.82±5.50、0.10±0.03,后囊膜下白内障组分别为:61.03±9.52、1.85±0.58、8.71±6.36、0.08±0.03,ANOVA检验各种形态白内障组在年龄、LN (OSI)、MTF (cut off),SR之间的差异均有统计学意义(F =15.17、22.49、12.38、11.84,P<0.05);经Bonferroni检验,年龄为皮质性白内障组和后囊膜下白内障小于核性白内障组;OSI值为后囊膜下白内障、皮质性白内障及核性白内障依次减小;MTF cut off为后囊膜下白内障、皮质性白内障及核性白内障为依次增大;核性白内障的SR值比皮质性白内障、后囊膜下白内障大;皮质性白内障、核性白内障及后囊下白内障的BC-VA与OSI均呈正相关(r=0.41,P<0.01;r=0.33,P=0.04;r=0.32,P=0.05).皮质性、核性及后囊下白内障的BCVA与MTF cut off均无统计学意义(r=-0.26,P=0.08;r=-0.09,P=0.58;r=-0.18,P=0.29).皮质性、核性及后囊下白内障的OSI与MTF cut off相关性均差异有统计学意义(r=-0.88,P<0.01;r=-0.70,P<0.01;r=-0.76,P<0.01).结论 散射光、MTF (cut off)、斯特列尔比可以客观地反映白内障形态对患者视觉质量的影响,后囊膜下白内障对视觉质量影响最大.

Objective To explore different cataract morphology on the visual and optimal performance.Methods Cataract patients who visited the Eye Hospital of China Medical University from March 2013 to August 2013 with a best corrected visual acuity (LogMAR) of 0.3 or better,were classified into 3 groups:cortical cataract group (44 eyes of 41 subjects),nuclear cataract group (39 eyes of 36 subjects),posterior cataract group (38 eyes of 36 subjects),BCVA were measured with phoropter,objective scatter index (OSI),MTF cut off and Strehl ratio (SR) were measured with OQAS.Results The best corrected visual acuity (LogMAR) were 0.23±0.10 in the cortical group,0.23±0.08 in the nuclear group and 0.25±0.10 in the posterior group,BCVA did not differ significantly for cataract of the 3 types (F =0.68,P =0.51);The age,LN(OSI),MTF cut Off and OR were 64.57±6.25、1.35±0.63、11.85±4.24、0.09±0.02 in the cortical group,69.90±5.01,1.01±0.43,14.82±5.50,0.10±0.03 in the nuclear group,61.03±9.52,1.85±0.58,8.71±6.36,0.08±0.03 in the posterior group respectively (F =15.17,F =22.49,F =12.38,F =11.8;P <0.05);In Bonferroni test,OSI,MTF cut off and SR were significantly different in cataracts of different morphologies (P <0.05);Age of cortical cataracts was older than that of nuclear and posterior cataracts;From nuclear,cortical,posterior cataracts OSI deteriorate in order;From nuclear,cortical,posterior cataracts MTF cut off decreased in order;SR in nuclear cataracts were significant better compared with that in cortical and posterior cataracts (P <0.05);no significant differences were found between the cortical and posterior cataracts (P >0.05).Correlations between BCVA and OSI were r =0.41,r =0.33,r =0.32;P ≤0.05) in cortical cataracts,nuclear cataracts and posterior cataracts (P >0.05);however,BCVA had no significant correlation with OSI in any cataracts (P >0.05).OSI correlated with MTF cut off in any cataracts (r =-0.88,P <0.01;r =-0.70,P <0.01;r =-0.76,P <0.01).Conclusions OSI,MTF cut off and SR can objectively reflect the influence of cataract morphology on quality of vision,and the influence is most notable in posterior cataract.
