目的 探讨眼内镜(ophthalmic endoscopy)监控下巩膜层间连续套环缝合睫状体复位术治疗外伤性睫状体分离的效果.方法 对11例(11眼)外伤性睫状体分离,应用作者设计的眼内镜监控下巩膜层间连续套环缝合睫状体复位术进行治疗,术中以眼内镜随时观察睫状体复位情况.术后应用散瞳剂和皮质类固醇控制炎症.结果 所有患者术后1周UBM检查睫状体均复位.10眼术后1周内眼压恢复至13 ～22 mmHg(1 mmHg =0.133 kPa),1例术后3个月恢复至21 mmHg,术后末次随访眼压平均为(17.18 ±0.32) mmHg,与术前眼压(7.64 ±4.41) mmHg比较,差异有统计学意义(t=5.70,P=0.000).未发生医源性白内障.结论 眼内镜下巩膜层间连续套环缝合睫状体复位术治疗外伤性睫状体分离,疗效确切,安全.

Objective To investigate the efficacy and safety of continuous collar suturation in ciliary body reduction surgery (cyclopexy) for traumatic ciliary body separation (amotio corporis ciliaris)under the monitoring of the ophthalmic endoscopy.Methods Eleven eyes of 11 patients who had traumatic ciliary body separation were treated.The method of surgery was the continuous collar suturation in sclera surgery for traumatic ciliary body separation under the monitoring of ophthalmic endoscopy,and the ciliary body was observed with ophthalmic endoscope in any time.Results One week after the operation all the cyclodialysis were reseted by examination of UBM.The intraocular pressure of 10 patients were normal (13-22 mmHg) within one week after the operation.The intraocular pressure of 1 patients was normal (22 mmHg) three months after the operation.No iatrogenic cataract happened.Conclusion Continuous collar suturation in sclera surgery under the monitoring of the ophthalmic endoscope is efficient and safe for traumatic ciliary body separation.
