目的:调查全膝关节置换术(TKA)中吸引头细菌的污染率。方法对2015年2月至2015年5月在广州医科大学附属第一医院手术室行TKA手术的患者43例52膝进行对照研究,使用两套吸引器负压装置,在无菌区吸空气的吸引管头为对照组,手术中常规使用的吸引管头为实验组。对术中使用的吸引管头进行拭子细菌培养,统计细菌培养的阳性率。结果对照组术中1h及术后吸引头拭子培养无菌群生长;实验组中有2例在术中1h、术毕均培养出相同的表皮葡萄球菌,污染率为3.8%。两组比较无统计学意义( x2=2.039,P>0.05)。结论在本研究中,TKA术中吸引头细菌污染率低,建议不需要强制常规每60分钟更换吸引管头。

목적:조사전슬관절치환술(TKA)중흡인두세균적오염솔。방법대2015년2월지2015년5월재엄주의과대학부속제일의원수술실행TKA수술적환자43례52슬진행대조연구,사용량투흡인기부압장치,재무균구흡공기적흡인관두위대조조,수술중상규사용적흡인관두위실험조。대술중사용적흡인관두진행식자세균배양,통계세균배양적양성솔。결과대조조술중1h급술후흡인두식자배양무균군생장;실험조중유2례재술중1h、술필균배양출상동적표피포도구균,오염솔위3.8%。량조비교무통계학의의( x2=2.039,P>0.05)。결론재본연구중,TKA술중흡인두세균오염솔저,건의불수요강제상규매60분종경환흡인관두。
Objective To investigate the bacterial contamination rates of suction tip in total knee arthroplasty ( TKA).Methods Fifty-two knees undergone TKA in the operating room of the First Affiliated Hospital of Guangzhou Medical University from Feb 2015 to May 2015 were enrolled in this controlled study.Two sets of suction tubes were used in every TKA procedure, of which one was used in the control group without contacting the surgical field and the other was used routinely in the experimental group.Specimens were taken by swabs and sent for culture from inner wall of the suction tips one hour after the incision and at the end of the surgery.The bacterial contamination rates of the suction tip were compared between the two groups.Results In the control group, no flora was found in the laboratory.In the experimental group, the samples from the two suction tips presented positive culture results with identical staphylococcus epidermidis both one hour after the incision and at the end of TKA. The contamination rate of the experimental group was 3.8%, not significantly higher than the control group ( x2=2.039, P >0.05).Conclusion As the low contamination rate of suction tip in TKA, it is not suggested suction tip changing in every 60 minutes as a compulsory sterile protocol in TKA.
