目的:探讨中药丹红对早期 CKD 大鼠肾脏病理损伤的疗效及氧化应激在其中的作用。方法:30只雄性 SD大鼠随机分为模型组(早期 CKD 模型),对照组(假手术)和丹红干预。单侧肾切除建立早期 CKD 模型,观察丹红干预后肾病病理改变和积分水平、24 h 尿蛋白定量和肾功能情况,检测肾皮质 SOD 和 MDA 活性。结果:与对照组相比,模型组肾小球轻中度系膜细胞增生伴局部系膜基质增宽、球囊壁黏连及毛细血管壁改变,肾小管间质不同程度炎症细胞浸润与纤维化;与模型组比较,干预组肾脏病变减轻;模型组总病理积分高于对照组(P ﹤0.05),而干预组积分低于模型组(P ﹤0.05)。模型组与干预组24 h 尿蛋白排泄量高于对照组(P ﹤0.05),且干预组较模型组排泄减少(P ﹤0.05)。三组血肌酐差异无统计学意义,但与对照组相比,模型组和干预组血清 CysC 浓度增高(P ﹤0.05)。模型组肾皮质 SOD 活性较对照组降低(P ﹤0.05), MDA 则增加(P ﹤0.05)。与模型组比较,干预组 SOD 活性升高(P ﹤0.05),MDA 含量减少(P ﹤0.01)。肾内 MDA(Ln MDA)和 SOD 活性与尿蛋白排泄量显著相关(r =0.436,-0.557,P ﹤0.01)。结论:丹红通过调节 SOD 和 MDA 活性,缓解氧化应激损伤,改善早期 CKD 大鼠肾脏病变。

목적:탐토중약단홍대조기 CKD 대서신장병리손상적료효급양화응격재기중적작용。방법:30지웅성 SD대서수궤분위모형조(조기 CKD 모형),대조조(가수술)화단홍간예。단측신절제건립조기 CKD 모형,관찰단홍간예후신병병리개변화적분수평、24 h 뇨단백정량화신공능정황,검측신피질 SOD 화 MDA 활성。결과:여대조조상비,모형조신소구경중도계막세포증생반국부계막기질증관、구낭벽점련급모세혈관벽개변,신소관간질불동정도염증세포침윤여섬유화;여모형조비교,간예조신장병변감경;모형조총병리적분고우대조조(P ﹤0.05),이간예조적분저우모형조(P ﹤0.05)。모형조여간예조24 h 뇨단백배설량고우대조조(P ﹤0.05),차간예조교모형조배설감소(P ﹤0.05)。삼조혈기항차이무통계학의의,단여대조조상비,모형조화간예조혈청 CysC 농도증고(P ﹤0.05)。모형조신피질 SOD 활성교대조조강저(P ﹤0.05), MDA 칙증가(P ﹤0.05)。여모형조비교,간예조 SOD 활성승고(P ﹤0.05),MDA 함량감소(P ﹤0.01)。신내 MDA(Ln MDA)화 SOD 활성여뇨단백배설량현저상관(r =0.436,-0.557,P ﹤0.01)。결론:단홍통과조절 SOD 화 MDA 활성,완해양화응격손상,개선조기 CKD 대서신장병변。
Objective:To determine the effect of traditional medicine Dan - Hong on the oxidative stress response and renal injuries in early CKD. Methods:Thirty male SD rats were divided into three groups(n = 10,respectively):early CKD group(Model group),sham - operated group(Control group)and Dan - Hong treated CKD(Treated Group). Surgical removal of the right renal was performed to build up rat model with early CKD,and Dan - Hong was administered for the treated group. Renal pathological le-sions were evaluated after HE and PAS staining,24 h urine protein excretion and renal function indexes(Scr,CysC)measured,ac-tivities of SOD and MDA in renal cortexes examined. Results:Renal sections in early CKD group showed varied proliferative mesang-ial cells,broadened measangium matrix,glomerular balloon adhesion,thicker capillary wall,interstitial fibrosis and inflammatory monocytes than control group. Renal lesions alleviated dramatically after Dan - Hong treatment. The model group had a larger gross pathological score of kidney damage than control group(P ﹤ 0. 05),but a less score than treated ones(P ﹤ 0. 05). There were more 24 h urinary protein excretion and higher plasma CysC levels in early CKD and Dan - Hong treated animals than in control group(P ﹤0. 05). Treated rats excreted less urinary protein than CKD animals without Dan - Hong injection(P ﹤ 0. 05). An increase in oxida-tive damage index MDA(Ln MDA)and a decrease in antioxidant enzyme SOD in the early CKD model was observed,as compared to sham animals(P ﹤ 0. 05). Upon Dan - Hong treating,activities of MDA significantly reduced and SOD up - regulated(P ﹤ 0. 05). MDA(Ln MDA)and SOD levels in renal cortex were associated with urinary protein leakage(r = 0. 436,- 0. 557,P ﹤ 0. 01,re-spectively). Conclusion:Traditional medicine Dan - Hong alleviated harmful effects by oxidative stress by modulating activities of MDA and SOD,led to beneficial change for renal pathological damages in early CKD rat model.
