目的：探讨植入型凶险型前置胎盘子宫切除的手术指征、时机及临床意义。方法对2009年2月至2014年10月佛山市妇幼保健院收治的术中行子宫切除的14例植入型凶险型前置胎盘患者的临床资料进行回顾性分析。结果全部病例均抢救成功，且术后恢复好，无感染及晚期产后出血等并发症。结论植入型凶险型前置胎盘是引起产后出血的主要原因之一，对胎盘植入面积太大无法保留子宫或出现无法控制的难治性产后出血，及时、果断地行子宫切除是挽救孕产妇生命的有效措施，对降低孕产妇死亡有重要意义。
목적：탐토식입형흉험형전치태반자궁절제적수술지정、시궤급림상의의。방법대2009년2월지2014년10월불산시부유보건원수치적술중행자궁절제적14례식입형흉험형전치태반환자적림상자료진행회고성분석。결과전부병례균창구성공，차술후회복호，무감염급만기산후출혈등병발증。결론식입형흉험형전치태반시인기산후출혈적주요원인지일，대태반식입면적태대무법보류자궁혹출현무법공제적난치성산후출혈，급시、과단지행자궁절제시만구잉산부생명적유효조시，대강저잉산부사망유중요의의。
Objective To study the indication, occasion and clinical significance of hysterectomy for treatment of pernicious placenta previa with placenta accrete.Methods Clinical data of 14 patients underwent hysterectomy for treatment of pernicious placenta previa with placenta accrete were retrospectively analyzed from Feb.2009 to Oct.2014 in Foshan Maternal and Childhealth Hospital.Results All the cases were rescued successfully with good postoperative recovery and free from infection and late postpartum hemorrhage . Conclusions Pernicious placenta previa with placenta accrete is one of the main reasons to cause postpartum hemorrhage.Hysterectomy is an effective measure to save the life of the mother in time resolutely for the placenta implantation too large to reserve the uterus and the occurrence of uncontrollable postpartum hemorrhage.It had great significance to reduce maternal death.
